To qualify as an assisted living residence in West Virginia, a facility must provide housing, personal care services, supervision, and some skilled nursing services to four or more residents. If a facility provides care to 4-16 residents, it’s considered a small assisted living residence. And, if a facility provides care to 17 or more residents, it’s considered a large assisted living residence.[05]

Assisted living residences undergo an initial inspection before licensure. Thereafter, facilities are inspected on a yearly basis. You can find an assisted living residence’s most recent inspection report using the Office of Health Facility’s Licensure and Certification’s Health Care Facility Lookup.
