Terrible

I have many many complaints. The phone does not get answered. I have to physically drive over to speak with anyone!! I'm not happy. They gave my papaw for dinner carrot soup 1/2 cup. And ONE slice of lunch meat on bread. Smh. I came there and it was absolutely disgusting.

No bed alarms??

Why in a rehab which includes many people who are there for or have a history of falling, would you not have any bed alarms whatsoever. My 94 yr old grandma was found on the floor 3x in one week in the morning. No staff sure of how long she has been there. My grandma was not confused and said one night it was hours. To top it off she was fine one day that I was visiting her, breathing/lungs sounding clear (I'm a nurse and notice these things), then 6 days later of myself not visiting, DIES of pneumonia, suspected aspiration pneumonia, [date removed]. MD stated the x ray looked terrible and not like new onset which means they must've noticed her sounding bad in the days prior and failed to do anything. In fact my mom who was there everyday pushed for an x ray the day she passed and their doctor claimed it "looked fine". Hospital MD was stunned he said that. She became septic and died 3 hours later.

Incompetent medical care - Don’t take your loved one here

This is a review of the “skilled nursing care.” My wife arrived at The Grand on a Friday afternoon, 10 days following back surgery. It took more than a day for them to get all her prescription meds in. There were no physical therapy or wound care nurse services on the weekend. Nursing was understaffed and not well trained. A doctor did visits in the mornings, usually before the facility was open to visitors so family miss these visits unless you get in early. I tried to communicate with staff about my concerns regarding my wife including confusion and weakness. The doctor did not make rounds at all on Monday so I was determined to speak with him on Tuesday. My wife passed away during the night, about three and a half days after arriving at The Grand and more than 40 hours since their doctor last saw her. In my opinion The Grand’s incompetent medical care was responsible for her death. I would never ever recommend this facility to anyone needing skilled nursing care for a loved one.

Disappointing

My husband was here after spinal decompression surgery. The staff was very nice but did not follow through with recommendations from the doctor. Medications were not given per doctor's request. At 2 weeks from surgery he was sent via wheelchair to doctor's office to have stitches removed. He had not sat upright for any length of time.He was dehydrated and had a very low blood pressure so he was admitted to the ER for possible sepsis which he didn't have.. When he was sent ho.e I was told he needed a lift and hospital bed which I declined. He arrived home via ambulance and was incoherent. He was again dehydrated and blood pressure was low. After 3 days at home, his blood pressure stabilized. He is able to transfer himself with little help from bed or chair to wheelchair. He is much stronger, oriented to himself and location. He is getting home PT and OT. Both women who evaluated him could not understand why he did not receive better therapy.

The Good, The Bad and the very much UGLY!

The Good, The Bad and the very much UGLY. I am about to post a very fair review of THE GRAND'S "SKILLED NURSING FACILITY" in Dublin. My wife suffers from Parkinson Disease and as a result of a fall needed a Skilled Nursing Facility to recover in 2019. I was given a Medicare. Gov. listing of Skilled Nursing Facilities in May 2019 to select a location. At that point The Grand was very highly rated. I will only discuss one category "Staffing" and it was "Above Average" for me to have selected that location. The current rating for "Staffing" at The Grand published by Medicare.Gov. is "Much Below Average". My wife was there for 7 months until [date removed], when she suffered a seizure and arrived at Riverside Methodist Hospital with a temperature of 102 degrees and PNEUMONIA in both lungs. Most of the Nursing Staff who I have met scheduled for the evening shift are undertrained at best. In seven month, The Grand, can only now be described as an "UNSKILLED NURSING FACILITY".

Too small for my brother although the bedrooms were a nice size. Meals are not included in fee. No nurse on duty 24 hours. No medical help really. Have to pay for extras. More like independent living.

Need more staff and better training

Family member did not improve and responsive rate was very slow. Was frustrating for us to try to get people to listen and more importantly to care about our requests and reason for being there. The stay did not accomplish what we needed and we disappointed with the level of expertise and compassion there.

Stay away

The facility is beautiful and clean. However, they are not competent to provide rehabilitation services. My mother was sent there to have therapy after getting a pacemaker because she was very weak. She told us she was walking and eating. Come to find out she was refusing therapy and barely eating. Not once did any member of the staff - from the doctor, head nurse, floor nurses, pt or social services - let my dad or I know. Only when they threw her out for not complying with therapy did we find out. She ended up back in the hospital for 2 plus weeks then went to another facility for therapy but it was too late. She passed away 4 days ago. I truly believe had The Grand contacted us - my dad went there every day for several hours to visit her - the outcome would have been much different. Please don't entrust your loved ones to this facility.

Think twice about this place

Stay far away! The worst place to send your loved ones trying to get back to independent living. My dad was there for 10 days never was seen by a doctor after falling twice due to no response from night shift staff. He was sent back to hospital after telling them if they didn't call for transport to the hospital I was calling the squad. He ended up back into hospital another week. The physical therapy department was great. But that is it.

No so good start, unprofessional, under st

Was a patient here one time after being discharged from OSUH . I was on solid food and liquid drinks, and had a voice box at discharged. Was checked into this place , first spent 6 hours staged down in a dark hallway because no room was available at the time, but they knew I was coming. Than they took me off solid foods and liquid drinks. When I did get a room night nurse came in and took my voice box and left my room with it and than lost it. There had some staff that were very good, very polite, and very helpful. But most were very rude very unprofessional, I was bed riding, could not move and needed help doing everything. One day in particular, I needed to get up and use the bathroom, pushed the call button, no response, ended up filling my shorts, pushed call button to get cleaned up, no response, ended up in bed with full shorts for the entire day and well into the night. Most of first shift, all of second shift and half of third shift. Now I'm talking almost 16 hours in shorts that I pissed four times and [language removed] 3 times. Sadly I would not recommend this place at all. At the end of my stay my insurance was ending and I was supposed to get a walker, a shower chair, a tool to help put my socks on, a tool for reaching. Now they knew my insurance was only good till the end of the month, after that into the next month I had no insurance and no money. Well they ended up keeping me on the schedule for therapy, and also knowing that friends were coming to pick me up. So at the time of check out into the next month, well no insurance, ment no Walker, no shower chair, no tools, and a $2,000's bill for 3 days into them month that I had no insurance and no money.

The Grand is awful!

The Grand has a beautiful facade but that is all. Terribly understaffed with those who do work there only willing to do the very, very minimum. many of these people suffer from dementia and are very vulnerable. There are very few activities to help stimulate their mind and most are left just to sit for hours on end. The food is awful and we saw many family members bringing take out food to relatives since what was provided was inedible. Nursing staff is all but invisible and mostly disinterested. There is no continuity with the staff and there seems to be a revolving door - hardly ever the same staff member. The aides were scarce and many times downright nasty -as were the few nurses we saw. They tout the ice cream parlor - it is almost never open, the "deli" nothing more than an upgraded vending machine, and the pub - also hardly opened. Most distressing of all was the lack of care- patients sitting in urine stained clothing because they simply could not wait any longer to be taken to the bathroom and missed medications. The raves you see posted on their web site are by employees and friends. Do not be fooled! If you are elderly, suffer from dementia or vulnerable run from The Grand!!

Trust me there is nothing GRAND? about The Grand of Dublin. It's all glitz and glamour. But the quality of service is just as substandard as any other basic nursing facility. We took our dad out of here due to too many issues to list. The social worker was one of the most inhumane we met and should have her license revoked. My Dad still has Medications that haven't been fulfilled yet. Would never recommend this facility to anyone else. Definitely filing complaints with the State and Medicare. I know of several? families who feel the same. Don't believe anything they tell you on the tour, the ratios are never what they claim and on many occasions we found our Dad wandering and [Removed], with green lights on for over and hour at times. Keep Looking, Do not send here!

After looking at several nursing facilities my father and I chose to have my mom stay here on a 5 day respite stay in the memory care unit. [Removed] took us on a tour of the facility; It appeared nice and clean, with a slight smell of urine in the memory care unit. When asked what the staffing was like on the unit she said there was one nurse and one aid, and that the nurse doubles as an aid and helps out doing aid duties due to the low acuity level of the patients and their low census of 15 patients on the unit. I asked her if they are to be taken to the bathroom every two hours and she said, "Of course, we will take great care of your mother, I know it is hard, but we will make sure she is taken care of." My dad gave them a list of the medications she was taking prior the the beginning of the stay. Once she arrived he asked if they had the list and they said they weren't sure, it may have gotten thrown away, they can't find it. I printed off the list and went over it with the nurse and they were missing one of her blood pressure medications, and when addressed said they would add it onto her list of meds. The second day my Dad went to visit her in the evening and she was still in her pajamas, they gave her Haldol for some aggressive behaviors and said they just didn't want to fight with her. My father asked if she had had a BM and the nurse stated that she could give her something to go. My father simply said she just needs to sit on the toilet for 10 mins and she will go. If you give her a laxative she will have an accident. I also inquired about her last BM on a visit and the nurse said, I don't know, thats something you'll have to ask the aid. When I addressed the aid, I asked about my mother by name and requested information about BM documentation. She put her hand on her chin and said, " oh, that one, that one, thaaaat one, no, i dunno, let me see as she was checking her computer. No BM was documented after 3 days. I simply took her to the bathroom and she had a BM, and I also cleaned up her shirt where there was several food stains on the front of it. On my visit I took my mom out into the secure courtyard and sat on a bench to visit. As I was looking out the gate was flapping in the wind, not so secure I'd say and I reported it to the nurse. I wanted to check up on her around 10:30 pm that night so I decided to call. The phone rang for 13 minutes before anyone picked up. They said they would have to find the nurse, and that they would go over to the unit and find her. The nurse was never found and the phone just kept ringing. The third day my dad found urine soaked clothes in the shower, they said she was incontinent of urine. Even her shoes were wet with urine.She has NEVER been incontinent of urine. This means THEY WERE NOT TOILETING HER EVERY TWO HOURS! Her teeth were caked with food, they were not helping her brush her teeth. My dad stayed there until 11pm and she had still not received her evening medications. My dad asked for a cup for some water for himself and my mom and the aid said she could not find any and would have to go to another unit to get some, but couldn't leave because she was the only one on the floor. My dad asked the nurse to come and give her medications. Apparently the nurse was on another floor passing medications. HOW can the nurse double as an aid if she is on another floor passing medications? When my mom finally got some water she downed a glass and a half! Have they even been giving her water? She was the first to get her medications on the unit at 11:02 pm. No one was in their pajamas and were all still up watching tv on the couch! My Dad was wondering what kind of place is this. He said, "I can give better care to her at home, I don't need this!" He decided to take her home at once. When faced with the complaints the nurse stated, " I'm sorry you feel this way." Think twice before sending your loved one here.The building may be nice, but thats all. I am a Registered Nurse and unfortunately they had me fooled...don't send your loved ones here.

this place has been horrible

This place is absolutely HORRIBLE, and for the most part so is the care. My grandmother is in this facility and i have had nothing but problems. Came here on Monday room was unkept and she hadn't been bathed and it was going on the afternoon. Exposed wires on one of her controls and stains splattered all over her bedding and even privacy curtains. She's bed bound and her drawers had open half eaten snacks. Another drawer has random pills scattered in the drawer in which no one can or even tried to explain or dispose of. I have made severalvisits and complaints inthe last 3 days. Tuesday afternoon half of my concerns went ignored. My grandmother is unable to do anything for herself. I showed up today...Wednesday...not only is the same old water pitcher dated yesterday still sitting here but she's experiencing spasms that you could clearly see the discomfort in her face. I addressed my concerns to her assigned nurse who responded rudely. I sat waitibg only to go back out and find that she left for the day without following up. I again addressed my concerns. My patience has expired with this place. 2 hours down and non of the 3 staff members i have addressed have come to bring her a clean water pitcher. I am in this momment contacting other facilities. I have pictures that i wish i could upload. At this point i wouldn't recommend leaving your pet goldfish here.

Never take a loved one here

This place is no good,they never look in on your loved ones till they see you coming,my father was in full rigamortis when I found my father,that means they did not,they never checked on hem four over 3 to 4 hours,but they told us they where in a half hour before I got there,my father was in bad pain one night,pushed call light to get he's Meds and pain meds they never came,I sat there for 3 hours trying to get help never came,if you love your family never take them here.
