Our mission is to provide quality home healthcare services to the elderly and disabled, including those confined to their homes due to temporary medical conditions, ailments, or aging.

We are committed to offering the support necessary for adults, and helping patients to deal with any unresolved emotional issues (brought about as a result of their newly diagnosed medical problems)which may probably interfere with both activities of daily living, life goals and aspirations.

Whether you are dealing with the stress and anxiety that is often felt in providing care for aging parents, the physically challenged and other loved ones in need of home care and support; at Grace and Mercy Health Services (GMHS), our compassionate home care team supports our clients and their families during this period. We also provide RESPITE care for families that need to take a break from caring for their loved ones in order to rejuvenate.

We provide home health services to clients whose primary payment method is through: - Medicaid - Commercial Insurance - Worker's Compensation - Managed Care Organizations - Private / self pay Our patients are admitted through referrals from the patient's physician, hospital case manager, discharge planner, Insurance case manager or the individual or member of the individual's family. Each patient admitted to the Agency must meet the requirements for homebound status and be under the care of a licensed physician. The admission process includes a complete home health assessment and identification of pertinent treatment processes with defined outcomes.
