PROGRAM STATEMENT Faith Family and Determination AFC has been established to provide Specialized Services to mentally III and developmental disabled co-ed’s ages 20 to 80.
Our staff here at Faith Family and Determination AFC will provide physical, emotional, intellectual, and social needs for each resident, be capable of appropriately handling emergency situations.
In home services will Include proper hygiene, supervision, personal care, protection, room and board, meals, proper attire, washing clothes, medication, how to fill out agency papers, shopping, spiritual illumination, weekly outings, how to go to the doctors office, how to use the transit system, how to schedule appointments, how to write a letter or check and safe sex.
GOALS AND OBJECTIVES Faith Family and Determination AFC offers assistance to help the consumer achieve self-sufficiency.
Our Program will provide a variety of services designed to support a successful transition from foster care to independent living.
Our Objectives are to maintain persons with disabilities in the community, coordinate and integrate health care of consumers, coordinate and supervise personal care financial, legal affairs, support and encourage utilization of community services and resources.
OUR MISSION Our purpose through a partnership between Government and State agencies In Improving and providing services to consumers in a home setting to achieve self- sufficiency, Our compassion creates the desire and wisdom for our staff to meet and exceed the care anticipated by our consumers.
