At Creekstone, we provide a multitude of community features. Residents can enjoy regular off-site experiences and day trips. Cats and dogs are permitted in private quarters. We offer free wi-fi. Free resident parking is available at our location. Free move-in is currently being offered. It's our delight to accommodate visiting guests, meals provided. Dining includes specialized dining programs available to residents upon request. Food service is available to residents at any time, day or night. We offer meal services such as low/no sodium options and low/no sugar options. Residents can exercise their physical and mental well-being with specially curated on-site activities. The exercise program includes stretching classes. We love to celebrate with holiday parties, birthday parties and BBQ's, picnics, and outdoor parties. Our inter-generational programs offer many benefits for both children and seniors, emphasizing the joys of human connectivity and propagating longer and happier lives for our residents. Our

communal space has a piano centerpiece for musically inclined residents and scheduled performances. For residents who are reluctant to participate in social or recreational activities, we offer pet therapy as a positive and stimulating alternative. Residents enjoy regular trips and outings to local neighborhoods and towns. We provide regular musical entertainment for the use of our residents - and in some instances, to take a trip down memory lane. Outdoor common areas are designed with accessibility, flow, and movement in mind. Our community grounds is comprised of walking paths that are accessible to seniors with a variety of physical capabilities, an enclosed-courtyard to enhance recreation hours, raised gardening beds available to residents during their leisure hours, and an outside patio garden accessible to all residents. Indoor common areas are a nice, quiet place to enjoy the company of fellow residents or just to relax. Our community is secured to ensure our residents are protected.

We offer studio units. Ground floor units are available. We have free wi-fi for all of our residents. To accommodate our residents, our community has WanderGuard capability, and air conditioning and temperatures are regulated to optimize comfort and safety. Resident rooms featuring roll-in showers are available. The facility also offers free cable satellite TV to all residents.

All of our residents have access to our transportation services at a reasonable cost. We have a beautician conveniently located on-site - residents can easily receive beauty treatments for all occasions. We offer devotional services on-site, so residents can practice or worship in the privacy of our community. Personal housekeeping services are provided as needed. Our convenient on-site beauty salon provides hair care and beauty treatments. Both laundry and dry cleaning services are offered. Protestant religious services are available, provided by a visiting chaplain. Our 24-hour awake staff is available to assist your loved one in fulfilling time-sensitive needs, should they arise. All services are delivered by licensed practitioners present on staff 24/7. There are licensed nurses on-site to administer injections when needed. Our medication-care managers organize the medication services available to our residents. Available services include on-site RN's and LPN's. Staff can engage in monitoring residents' blood sugar to tailor daily pill dosages. We have registered nurses on our community staff. We have a registered dietician / nutritionist on site. In addition to other transfer services, our community provides 1-person assisted transfers. Caregivers at our facility receive specialized, relevant training in a variety of areas. We recruit first-class visiting podiatrists whose job it is to ensure our residents are happy, fit, and mobile. Regular visits from specialized occupational therapists are available to residents to aid development, recovery, improvements, and maintenance of skills needed for daily living. Speech therapy services are provided by trained and certified speech-pathologists. A visiting nurse is available to all residents. We offer laundry and dry cleaning services as well. Grocery shopping services are available as needed. On-site hospice care is provided. Our community offers respite care - a short-term stay at a senior living community - perfect for those recovering from a hospital visit, the interim during which a caregiver takes an extended vacation, or simply to experience a senior living trial period. This community provides ADL assistance, insulin injections, medication administration, and licensed nurses are onsite to administer injections when needed. We can administer a sliding scale program to monitor insulin levels via glucose tests, and adjust medications based on the results. Services include incontinence care for bladder-related and bowel-related symptoms. Services for residents with Disorientation/Lack of Awareness issues ensure residents are well-directed (using queuing, for instance) and supported in a healthy lifestyle - nobody falls between the cracks. Alzheimer's and dementia may encourage wandering as a means of coping with anxiety. Our facility utilizes a locked and secured perimeter to ensure the safety of our residents. Our

facilities are secure and the staff is specially trained in de-escalating exit-seeking behaviors.

Our property is licensed for 60 residents. The building was built in 2016. We do not have age restrictions and accept residents of all ages. Local fire, police, and EMT services are nearby and ready to respond. The community contains several churches, shopping centers, and beautiful parks close by.

    We chose Creekstone because of the recommendations we received from others. The rooms are very good sized. Bathrooms are large and handicap accessible. The food is excellent, they cook it right there in the kitchen so its fresh and hot. It is served restaurant style. If there is something they cannot care for, they recommend something for us. My favorite part about the community would be the staff members, they become family and go above and beyond to take care of their residents.

    We chose Creekstone because of the recommendations we received from others. The rooms are very good sized. Bathrooms are large and handicap accessible. The food is excellent, they cook it right there in the kitchen so its fresh and hot. It is served restaurant style. If there is something they cannot care for, they recommend something for us. My favorite part about the community would be the staff members, they become family and go above and beyond to take care of their residents.

    We feel my mom is getting awesome care here. They are able to take good care of her. They are always on top of keeping the community clean. Especially during the Covid-19 pandemic, they have been on top of keeping the residents safe. The staff is very friendly, I feel that I can call at any time and they will answer my questions and are very welcoming. The care giving staff clearly is trained well and know what they're doing and on top of that are all compassionate. They have two activities directors that come up with things for the residents to engage in and the staff does a good job of getting the residents to participate. They have exercise classes, movies, reading time, and more. Creekstone is expensive, however we feel it is worth the cost for the level of care the residents are reviving. One of the most important things that we appreciate about Creekstone is the level of communication they maintain. They are very good about posting photos on Facebook, FaceTiming my dad so he can see my mom, and even providing a monthly newsletter.
