Both my parents have been in Church Creek during the years in Rehab and in Assisted Living. The facility is really slipping now. The rooms are not very clean, the staff is friendly, but not helpful and the care is sub-standard. The only shining star is [name removed] in Speech Therapy. If you are looking for a facility to really take care if your loved one, I suggest you look elsewhere.

Mom didn't like this community she felt like it was too busy and too large. It reminded her of being in a large city. The staff was friendly and helpful but mom would not have been happy there.

I saw a nice community when I had stopped by for a visit. They were doing a major renovation and it was looking very nice. It was not going to be a fit for our needs so we passed on moving in.

It is hard for me to talk about my experience at this community because everything was under construction. They where in the process of remodeling the place and re doing the dinning room as well as other common areas. The staff where kind helpful with the information they provided me though.

Mom is doing well Church Creek. The staff have been helpful and we have good communication with them. She likes the food especially the desserts. They offer daily activities and the community is beautiful. They invite family to enjoy meals with their loved one. I would recommend this community.

hi..my dad, rest in peace, recently passed and feel he would've lived for some time..10-20 years..longer had it not been where he went after breaking his hip..this charade known as church creek.. ..he wasn't on the nursing home side, but the healthcare/rehab side after breaking his hip..yet, i feel that if they permit this scathingly [Removed] behavior on the rehab side..then who is to say it would be any different in similar situation on the nursing home side.. ..quite frankly, i despise being the bearer of bad news and my mother doesn't believe in lawsuits, otherwise they would've had a monster one on their hands and [Removed], to boot..yet, my conscience compels me to shed light on this, to say the least..this unprofessional, exploitative way he was treated..yet, still feel he would still be alive today, if not for blatant disregard pertaining to the way 1)his hip stabilizer contraption hadn't been on the wrong way as directed..which, i wasn't aware of until almost a month into..subsequently, reducing his mobility after healing from it being on when it shouldn't have..ie. laying down and overnight causing his hip to heal up crooked..so when he came home he was pretty much wheelchair/walker bound and couldn't get the exercise that he needed for lots of reasons and deteriorated a lot faster from there. 2) awful bedsores from not being cleaned properly after defecating. 3) unnecessary tests ran on him [Removed], when they didn't run the tests they needed to, as a result not catching his liver enzymes skyrocketing (INR) test?..in which, his levels were around 30 and should of been more around 2.. ..i do realize he was a 'difficult' patient, but that doesn't give them the right to run frivolous money making tests instead to support sinister bottom line at the expense of the ones that should have, not to mention my dads life..very disturbing..we are grateful to have gotten him out of there when we did..December '15, or he would've passed two Christmases(tearing up) sooner.. ..its just very disturbing the lack of compassion and caring they had..maybe part of it was not enough staff, yet still doesn't excuse and find hard to believe..but still doesn't excuse at the expense of patients or should i say inmates.. ..they day he 'escaped' he was swollen like a watermelon in both legs(pitting edema)..all the way (50lbs overweight-water) almost up to his heart..this is very difficult for me..that it took a life threatening emergency to save him, at least for another year or two..because he wouldn't of made it if we hadn't of that day.. ..i can go on n on..yet, it doesn't make me feel one one bit better doing so..only adequate closure..

Dad did not like this community when we visited. While we were there it did not seem very active. The community was dated. The people we met were nice and the community seemed clean.

The activity level does depend on when you visit - we do frequent off campus outings and it can seem quiet at those times. Our Lifestyle 360 resident engagement program is based on the five dimensions of wellness with a full complement of intellectual, social, physical, emotional, and spiritual programs. Please accept my invitation to dine with us and to participate in a few Lifestyle 360 activities - we can review the calendar and pick out some that sound intriguing to you.
They padded our monthly bill with [Removed] charges. We are seeking legal remediation. The executives of this place are not only incompetent and uncaring, they are also shysters.

I looked at a room and it did not have a private shower. At the time I was looking for only primary memory care, They did not have focus only on memory care.

Church Creek does offer a wide range of services, including a dedicated Memory Care neighborhood (as well as Independent and Assisted Living options). We find that for some people it may take more than one visit to fully develop a feel for a community. As such we encourage you to come back for a second visit so we can spend some time with you to completely address any questions or concerns you may have.
When looking into long term care options for Mom location and the size of the apartment was our main concern since Mom is still at a point where she can do things on her own. When we found Church Creek it was great because the storage is right in front of her apartment and there's lots of room for her. The staff is very accommodating which we really appreciate. i would definitely recommend!

There have been a few minor issues, but other than that things are going well at Church Creek. The staff is attentive and accommodating and the facility is dated, but clean and well taken care of. They take good care of the residents.

I think that the facility itself is great; it has a great indoor environment. The food is wonderful. I think they probably could have somebody available to help new residents get acclimated more, like encouraging them to participate in activities.

I am worried all the time. She is having problems adjusting. Some of the people are very nice there, which is good, but the accounting department has not been good, like billing and all that sort of the thing. I feel like she is being taken care of, but I don't feel like they are that accommodating to the family. I just feel like i'm in the dark about somethings. It's a nice place, but it hasn't been the best. There just isn't enough feedback.

At this point they are doing very well. I don't have any major concerns. It's very clean, and the staff is friendly. It's almost like a hotel or a resort. It's still new, but so far, I am very happy with it.

They do an amazing job at Church Creek! There is absolutely nothing that they don't do well! Their sales team is amazing, and was so helpful throughout the whole move-in process. And they have a really excellent home health care service as well. All of people here, both the residents and the staff, seem very friendly and happy to be here. They are also very reasonably priced for what they provide, and there is no buy-in fee, it is strictly rent based. We would highly recommend them to anyone looking into senior living!
