Always clean, full of life and laughter
This place is like no other. I have seen many Skilled Nursing Facilities and this is the one you can walk in any time, day or night and there is no smell, its always clean, and the staff and patients are all laughing together. They interact and its not sterile and false and clinical. The staff genuinely connect with each other and the patients. The food is good, the management team is always helping, always attentive.

Horrible rehab
This place only cares about the money. Did not check my diabetic husbands blood sugar. Did not give medicine at proper times. Did not set up his CPAP. Did not get him up except for PT or OT. Long time after pressing the nurses button before help arrived.

Great Physical Therapy
Our loved one only stayed there for about a month while we were transitioning to go to another community. Meridian Center overall did a good job, and the finances were explained well of what we would be charged. The care was great at the beginning, and then they started being a little less attentive. They provided excellent physical therapy and that was the main reason we were there. They had some employees that just went above and beyond their regular duties. Overall it was a clean facility. There were times that we had to remind them to clean up in her room. There were activities, but our loved one was not able to participate at the time. The food was okay, but I felt it was an extra burden on the nursing staff that we had to order through them and then they would bring it to you, versus us going to order it ourselves. It did seem like they were a little short staffed on the weekends. The landscaping was beautiful and well-maintained.

This was awful.
This was awful. They kept coming up with excuses why they couldn't get things done. Even the corporate office wouldn't do anything about it. She would forget she ate, and they would just feed her an extra meal, so she would eat like 5 meals a day. They would lose hearing aides, and not take care of her personal needs to the point of infection. I constantly complained to the Administrator, and they never did anything either, and the corporate office didn't seem to care. They had a high turnover, and the staff never seemed very well trained.

A Place for Mom has scored Creekside Transitional Care and Rehabilitation 8.9 out of 10 using our proprietary review score.

We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.

Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
