They are doing a good job protecting the residents especially during the Covid-19 pandemic. The staff is very caring and they are all very respectful. My grandmother is being cared for well. One of the care takers in particular goes above and beyond to care for my grandmother. Previous to Covid-19, they offered a variety of activities available. They are very affordable for the level of care especially considering how well they take care of my grandmother. We cannot say enough good things about the staff and care here!

Terrible Place!
My father-in-law was admitted to Heritage Park (HP) on [date removed]. He was discharged from San Antonio Hospital in Upland after undergoing a partial hip replacement after falling and breaking his upper femur. 3 days later visitors were banned due to Covid-19. Two weeks after he was admitted, my sister-in-law met with his care team, including a doctor. He had not even started physical therapy! His care team was non-existent. He was not given any medication for pain until we inquired about this. They then started him on Oxycontin, and he was drugged up for the next 30 days! He was incoherent, could not carry on a conversation, and in fact would fall asleep in the middle of a phone call. The staff is a joke, and is constantly redirecting to someone else who is never available to answer questions. They discharged him when Medicare ran out, not letting us know that he was eligible for a medical transport to another facility. He could not stand on his own, could not transfer to a toilet, could not do anything for himself, and yet they threw him out, with half a tank of oxygen for his COPD. Do not use this place if you have other options. Terrible treatment of patients. By the way, now that he is off the Oxycontin, he is regaining his wits and can carry on a conversation.

dangerous situation
I would not recommend this facility to anyone. The care is extremely lacking. The air conditioning will not go below 85. The carpet is torn in areas where the residents walk and is causing a trip hazard. The turkey dinner consists of a slice of lunch meat with gravy. The night staff is unavailable to families and to residents. When you get to talk to the nurses they smile and say they will put a work order in but it has been 4 weeks and no changes. I do feel as if a few of the nurses care however they are understaffed and not the ones that care for the air or preparing the food. The care is completely unacceptable.

• HPNC lost Mom's phone • HPNC took 7.5 hours after doctor’s orders to place in IV in my Mom. In fact it required family intervention late in the evening to demand the IV to be started. • Water was kept out of her reach • Leaving breathing treatment apparatus on way after treatment done. • Did not provide alternate meals to food she could not eat leaving her without food and hungry • Nurse gave inaccurate information about when blood test was done. She said it was done when not done. • Never got her out of bed. • For 8 hours her iliostomy bag had been leaking and she asked several times that it be cleaned and changed. She laid for over 8 hours in her urine and feces. The result was a fungal infection that had to be treated at a local hospital. • While in HPNC’s care she was given Robitussin DM-Dexthomorphan and Guafenisin, which in combination with her other prescriptions may have cause her hallucinations. • Family “RESCUED” my mother from HPNC's care. HPNC refused to provide any and all documentation upon release with respect to here care, which is required by law.

Great Physical Therapy
My mom has been in there like 3 times, recovering from different things. The physical therapy department was wonderful, I felt. Some of the nurses and caregiver were really really good. The facility itself left a lot to be desired. The food was good but they really had an air conditioning problem. It was very hot when I went to go eat with her but the food was good and like I said the physical therapy was wonderful. There were a few nurses that I wasn't sure about but overall it was good. Outside they had a nice outside are but they had no umbrellas so you were just sitting directly in the sun.

Heritage Gardens
My mom has been there for about 3 months. They are taking really good care of her, the staff is awesome, and she loves the food. I looked at a lot of facilities in the area. I was looking for a home like facility, not too big or expensive, with activities to keep her busy. They are all of that and more. I have talked to other families with a mom or dad there and they all seem very satisfied with the care their family members are receiving. I am glad I found Heritage Gardens.

about Heritage Court Assisted Living

A Place for Mom has scored Heritage Court Assisted Living 8.5 out of 10 using our proprietary review score.

We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.

Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
