Alzheimer’s Memory Care
At Autumn Leaves, memory care is all we do. Autumn Leaves was founded when members of our family struggled with the same challenges you may be facing now: finding quality care for a loved one. We have designed our communities, from the ground up, specifically to meet the needs of those living with memory loss.We don’t simply care for people with Alzheimer’s and dementia; we nurture them. Our licensed nurses and staff are on-site seven days a week, trained specifically in memory care. We carefully design our buildings to allow safety and ease of movement, with wide hallways, abundant natural light and bright, airy decor. We prepare nutritious, appetizing meals tailored to individuals’ preferences and served in a homelike atmosphere. Our Inspired Connections Program™ keeps residents active and promotes well-being and socialization. We understand that memory impairment not only affects the resident but also the entire family, and that caregiving does not end when a loved one moves into our community. We work with and support our families. We rely on family members, the people who know each resident best, as part of our care team to help us get to know each person individually. At any of our 30+ family-owned and -operated communities you can feel confident that your loved one will receive unparalleled, compassionate care. "With your help we were able to make an informed
decision as to a facility which meets the needs of our dad and
family. We greatly appreciate your assistance and the
referrals you provided."

"Your service is invaluable. It helped me tremendously. I went
from a social worker at the hospice telling me on Monday they
wanted my mom out by Wed and handing me a list of 60 - 80 names to
evaluate on my own, to being able to tell you about my mom's needs
and getting a small list of known good places in about 15 minutes.
I appreciate you referring me to a list of 3 - 6 that you knew may
be appropriate, with good ratings by someone like you and families.
What a relief it was to have someone with your knowledge share
information to help steer me in the right direction and help me
make an informed decision.
