Sand Sage of West Plains
We chose Sand Sage because it was close to our home, the price was within our budget, and the unit was roomy and well-kept (fresh paint, no damage, like-new carpet, enough storage space). The staff I interacted with were most helpful and seemed to really care for my mom.

Feels like home
I loved Sand Sage immediately and the more we’ve gotten acquainted with the place and staff the more we love it. It’s a small community and that has really been great for my mom. The rooms are cozy and all on one floor with dining room, common area, and even courtyard outdoor space all a short walk away. The staff is amazing - super caring, always eager and quick to help with anything we’ve asked. Activities are low key - karaoke, trivia contests, Walmart trip every Friday. It all combines for a great experience that my mom says, “feels like home.”

Has room for improvement.
At the time my mom was there it was just post-covid. They were very short staffed and all the managers and nurses were being replaced. They were short staffed and took a long time to receive medication or for anyone to come when button was pushed. She was in a wheelchair and wore out easily. A lot of times she has to push herself to and from her room to the dining room. She liked all the food for the most part. They were accommodating to her diet restrictions. They had a lot more activities than the previous place she was at. I am sure once they were switched over to the new company they were able to retain employees. That was our main issue. Cost was better than most places we visited.

We are very happy
From our first walkthrough to moving in to mom living there we have been very pleased with our experience. [name removed] and [name removed]have been very helpful and very welcoming. The facility is always clean and inviting. We have met some of the residents and they were so nice to mom and to us. I don’t think I could have found a better place for mom.

Highly recommended
I still am no closer to the VA assistance. This week Sand Sage has arranged for several of us to speak directly to a VA representative so that we can finally get the assistance we all need. The VA representative is coming here due to the concern the staff has for our wellbeing. None of us have been able to get results on our own. They are caring and loving people who do the best they can to make our lives easier and happier. They address physical and mental issues rapidly.

Review for Sand Sage of West Plains
The staff were/are friendly and try to meet the needs of residents and there are various activities for the residents. Also, another reason I selected this place for my brother is it is only a few miles from my place. I had been in side this place 4 years a few years ago, and it was a nice place and not too many residents, didn't smell when you first walk in. When I toured this place in March of 2023, the facility had been updated, new paint, new flooring and new wall decor which was very eye appealing, no bad odors when you walk in. The facility has only 36 residents. It's nice that people can get to know each other and staff can identify residents, too.

Our experience
They are going through a lot of changes (TOTAL management change in the last 2 months). They said a lot of things when we signed up, that really hasn't been accurate. Staffing has been a issue for them. Short staffed means the residents don't get the attention they may need. My mom does like the food better than her previous facility. The cost savings, for us was worth the move.

This facility is good. They're doing some remodeling right now, so things are a bit more of a mess than usual. But that's understandable. The staff is friendly, I like them. They do offer activities, but my loved one just doesn't participate. The food is good, but they don't give a menu for the residents to know what is being served for each meal. There also aren't any options, they serve one meal for each breakfast, lunch, and dinner. No choices, whatever they serve is what the residents get. My loved one isn't thrilled about that, but still says the food is good. This facility is a great option for someone who is on a limited budget. I would recommend them to friends and family.

I toured this community and it was nice. It's clean, the staff was very friendly, our tour was nice. The only reason we decided on a different facility is the location, this one was further from home than we wanted.

A charming community
I was pleasantly surprised when going to Emerald Square. I couldn't believe how pretty the interiors were as well as the friendliness of the staff. They are much more updated than others in the price range. The cottage homes are spacious and well appointed.

Tips for evaluating your community options

Safety is a primary concern when seniors and their families look for Assisted Living. To ensure that a community meets your needs and expectations, we recommend that seniors and family members:

  • Tour the location: Touring allows you to speak to current residents and staff, and helps you confirm the accuracy of the community description and photos. Use our touring checklist to help guide your visit.

  • Confirm the availability of required services: Before you commit, make sure that the community offers all the services your senior needs, and that those services can be provided within your budget.

  • Know the Oklahoma regulations for Assisted Living: Each state has its own laws and inspections processes to ensure seniors reside in a safe environment. Depending on your state, you may even be able to look up inspection reports for the community(ies) you’re considering. Learn more about Oklahoma regulations for Assisted Living.

A Place for Mom has scored Sand Sage of West Plains 9.5 out of 10 using our proprietary review score.

We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.

Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score.
