We had a very bad experience, they were very disorganized. They took away the buzzer to get in so staff had to come let you in and let you out, they were short staffed. This upset a lot of the families and the residents. I think it would be helpful if they added a door bell. They could not get her medications straightened out and could not handle her. I continually got phone calls to calm her down and talk to her. Then they got on a kick of sending her to the hospital. There has been a huge turnover of staff, it is all new staff since we moved out so hopefully things are improving for the residents who are still there.

The original staff and original help are phenomenal but unable to keep that staff around. So they are cycling through staff too quickly. Having same staff stick around, routine is key. The meals and dining program is absolutely excellent, very good with no issues. Everything is offered with very large portions. They have a hydration station always available. They have a fantastic activities program and have recently hired an additional assistant. They participate in all National Food Days and National Days. They are currently on a Cruise for Hawaii and Italy upcoming this week. Very wonderful program. One of the nurses was asked to do something and she stated "We are not a skilled Nursing center". That was the only downside had at the current time.

They don't have adequate staffing, and they have agency staffing they are using. My loved one was left in bed until 10 am daily. The community wasn't able to provide a pureed diet, they classified it as pudding and applesauce. Most of the meals my loved one had was ham and cheese sandwiches. They tried to feed my loved one a full meal. They staff didn't watch my loved one close enough and my loved one fell in their care. There was never any staff around. The administration wasn't supportive of the concerns or questions about my loved one. They kept repeating that they would get things sorted out and they never did, even in the short amount of time my loved one was there. They told me that I was expecting to much. I ended up moving my loved one out of the community quickly due to the horrible experience and canceled the check. I ended up being at the community more than I wanted to be to watch my loved one. I was having to call in sick to work because of the concerns I had. I wouldn't recommend the community at all.

This community is not very crowded at all which we really like, it is clean and everybody seems very well cared for. Every time we visit the residents are clean, well dressed and participating in the activities. Overall it was a very nice first impression.

Very nice facility. [name removed] (executive director) was very pleasant and passionate about her work and Autumn leaves. She also spoke very highly of her staff of providers & caregivers and how passionate they were as well. I witnessed this for myself as we toured the memory care facility. The facility itself was very clean and modern (single story building). Plenty of scheduled activities for the residents, with many different options. The only drawback was that the rooms did not have a private shower. They did have a toilet and sink (1/2 bath type). This is an important item for my mother so we probably will look to another facility. Overall a very nice facility!

I like the staff very much. The food looks nice and the one time I ate there I thought it was good. They have activities all the time. They keep the community clean and well taken care of. This memory care community has codes that visitors need to use to enter and leave the building. I would recommend this community.

I have no complaints of my tour of this community. The staff where on time to our appointment and where very friendly to my father and I. They gave us a very detailed tour of there facility and made sure to answer and questions that we had for them in detail. All of the residents looked happy and content in there living environment which was pleasant to see. It was just not the right fit for my dad and we ended up choosing a different place for him.

I was not happy with the care here. They did not have a alarm for my mom for her to get some help. They did not have a bed alarm for here when she needed help.They did not do alot to stimulate people. Since my mother came out of the community the staff has been calling to see how she is doing. My mom had fallen and I was not called right away. They did not have good communication skills with each of the staff members. My mom needed skill living. I would not recommend this community to others for care for a loved one.

Dad is still adjusting to living at this community. We are happy with the staff. They are interactive and encourage him to participate. He likes the food and they offer a good variety. They offer daily activities. They keep the community clean.

There are some wonderful staff people at this facility, but they are overshadowed by many more who are not properly trained or managed. The leadership is nonexistent, and communication is terrible between management and medical/care staff. The public areas are clean enough, but resident rooms may not be cleaned appropriately. Have seen dried waste left in resident areas on many occasions. If your person has more than moderate dementia symptoms, look elsewhere.

Autumn Leaves is really a very nice community for seniors that are willing to cooperate. Because my Dad does not wish to be there he is not completely happy with the place but my experience with them have been great! Autumn Leaves has plenty of activities for their residents and I haven't heard any complaints about the food. I'm hoping that with time things get better for my Dad so that he can actually enjoy everything they have to offer.

As distressing as it is for us to hear about your dissatisfaction, we’d like to thank you for bringing your concerns to our attention. We have been in business for more than 14 years and strive to provide the highest quality care for our residents with Alzheimer’s and other forms of dementia.
Seems like a high quality facility. My mom has been there for just over 6 months. There was an issue when a few of the senior people left (I don't know why) about a month after my mom arrived. Before a new supervisor came in, my mother's care went down dramatically in quality for about a month or so. So much so that we actually considered moving her to a new place. She was not clean, not given her one-pill-a-day medication, was wearing other peoples clothes and all her socks were missing. Plus we received no communication what-so-ever from the staff. My mom does not need too much supervision, just help get her ready in the morning, and help get her ready for bed at night. But we waited before moving her and I'm glad we did. A new Supervisor came in and acknowledged the many problems and discussed our particular concerns. The issues that concerned us were taken care of, and from what we now see the facility is back to running efficiently and we feel fine with our mother being there. She seems to have adapted well and generally enjoys it there.

When searching for a memory care community for our mother, we compared pricing. Our mother needed help because of her memory issues and Autumn Leaves of Orland Park provides care for an all-inclusive price. While we felt the price was expensive, you get what you pay for and we feel the value is definitely there for our Mom. The staff is very attentive and are open to our constant contact with them. There's a doctor who comes to the site for individual care. The staff provides medication management and most of the rooms are private. Because Mom has been a fall risk, we feel that she is in a very safe environment. She even receives physical therapy. The community has population of about 40 residents and looks like a beautiful mansion. The are large rooms, sun rooms and alcoves throughout. We used caretakers prior to finding Autumn Leaves, who didn't work out. We are so happy that we were able to find such a wonderful place for our mother.

Cleanliness has gone down since my mother first started living here 2+ years ago.Her room now has to be asked to be cleaned. Her caregivers are angels, besides a few rowdy exceptions, who are also new. I visit more at night and let me say dinner is horrible!! The \\chef\\ Marlon fights with me about what I would like for mom and it seems like the CNAs or nurse have to provide a dinner for her, as they go into the kitchen once I ask for an alternative! Although I can say I love the friendliness from the front office and the care staff, but like I said a few new exceptions are a little rowdy and rude to patients and family!! Activities are great, my mother is engaged and loves Michael.
