My ambulatory and pleasantly forgetful Mother has been at StoryPoint for two years. It has been a journey for Mom to embrace "institutionalized" living. She was verbal in her disapproval to embrace her new lifestyle. After two years of constant attendance from the staff and nurses, Mom talks appreciatively saying, "they take good care of me and everyone is always nice to me". She may not recall names, but Mom is now ready to sincerely offer her gratitude for all those caring for her. I have great confidence in the StoryPoint management, administration, and all care givers. Mom has her personal hygiene needs met, her apartment is routinely cleaned, her laundry has been serviced. As Mom progresses in her aging process, I have been able to request more assistance for her needs. I am pleased with choosing StoryPoint as it is not just an "institution" for my Mom, but a community of caregivers that bring quality living to the residents that now call this place their home.

My wife and I decided to move my mom to Story Point Assisted Living in June of 2021. We chose Story Point because it was much more patient centered. The staff really cares about my mother. From the receptionist, to the nursing staff, to the maintenance staff, to the administrators they know who my mother is and act promptly to meet her daily needs. They design daily activities that encourage participation and cognitive stimulation. Mom is very happy about the menu choices that are offered at meal times. There are weekly bus trips, shopping excursions and even sightseeing tours. There’s a weekly movie night in a real movie theater- complete with popcorn! Mom enjoys making her own hairdresser appointments and she really likes the weekly opportunities to attend religious services. Story Point has been a wonderful experience for my mom. She has her sense of humor back and she is enjoying her life again. The patients’ needs come first in this facility and I am thankful for that everyday!

Mom is very happy here, it is a smaller facility with a very homey setting. [name removed] the marketing lady is awesome, we are there because of her. she gave us great feedback in the beginning and answered all questions we had. [name removed] the nurse my mom absolutely loves and she is also very happy with the meals as well. There is a really good mix of men and women who live here which is what she was looking for. I don't have to worry about mom anymore and have great peace of mind with her being here. Overall it has been a really good experience and we would highly recommend them to others who are looking.

I chose Brookdale South Bend because I was impressed by the way I was approached when I went here to tour. The people who answered my questions were very professional and thorough. I had confidence when I walked out that this was the place I wanted to send him to. The rooms here are private, it was very adequate for his needs. The meals were great and the portions were perfect size. Overall it was a great place to go, if I ever need it for myself I would go here with no question.

Heritage Point is a very clean community we have had no complaints so far. The staff members and the care services are wonderful. Sometimes our loved one is not a fan of the food, and if he doesn't like what they are serving they get hot dogs or grilled cheese. We have been so thrilled with the activities so far, they take the residents out on small day trips.

My mother was a resident at AH Kentwood until she passed away in April 2018. Sadly she was only there for 14 months. She was in the Riverstone Bldg in memory care. From the moment we made contact to her final breath this was truly a fabulous place. The staff was very accessible. I lived out of town and the team was great in keeping me up to date about my mom. She was as happy there as she would have been anywhere. We made a donation of something in error, [name removed] (the now ex-executive director) is no longer there to text which is why I am now searching for someone to speak with. The nice thing about my quest is I get to add to reviews long neglected.

Assisted Living

An assisted living community is a residential living community which offers support, assistance, and supervision to healthy individuals who are not in need of skilled nursing care. Assisted living communities usually offer supervision of care, help with appointments and transportation, and chances for social activity. In select situations, communities may provide respite, hospice, or dementia care. In Michigan, the annual median cost for an assisted living community is around $48,000 . The state of Michigan maintains a number of stipulations associated with this type of community. The Department of Human Services licenses regulates this community type. This community type has a number of provisions pertaining to medication assistance. These provisions include medication supervision and administration, proper training of direct care workers to handle and administer medications, and reminding a resident to maintain his/her medication schedule in accordance with the prescription. Staff are required to obtain training in a number of areas. These areas include standard precautions, first-aid, staff training program based on the residents’ service plans, and reporting requirements and documentation. This community has a number of provisions concerning food and diet which include three daily meals and snacks must be provided, Food must be prepared in accordance with the recommended daily dietary allowances of the Food and Nutrition Board of the National Academy of Sciences National Research Council, and communities must provide medical nutrition therapy if prescribed by a licensed health care professional. The minimum number of residents for this community type is 21. This type of community is required to offer a number of services. These include reminding resident of important activities to be carried out, supporting a resident’s personal and social needs, being aware of a resident’s general whereabouts, and assisting a resident to keep appointments. Before a resident is accepted, they must go through a service plan screening to guarantee their needs are going to be met. This screening includes a number of areas of interest such as service plan identifies the individual’s need for services, The service plan must be updated annually and following a significant change in health status, service plan takes into account the preferences and competency of the resident, and The home must complete a service plan in cooperation with each resident and/or their representative. Public assistance is available through Medicaid State Plan.
