The Ups and Downs of Sylvan Crossings

It's always good to start with the positive. My mother enjoys the CNA staff. They listen to her, provide needed assistance when asked, and give her a welcoming feeling. They are warm to her. I believe that the frontline staff in any organization are the ones who know the most, who can either create a welcoming atmosphere or a hostile and/or neglectful one. Sylvan Crossings has a pleasant atmosphere made possible by the CNAs. The setting is pleasant, as pleasant can be in such an environment. It's clean, has nice and well kept furniture. Management staff do try to listen to family members and have been helpful during trying and confusing times. We appreciate that. Areas we believe would benefit the residents if improved. First, the food quality is mediocre and the menu needs more attention. Hot dogs and bologna are made of highly processed meat. Not good when pursuing good healthy eating. Vegetables daily (cooked/uncooked), fruit daily and whole grains help to move food through the GI system. I saw very little of all of these items. Yes, the older one gets the less one tastes their food, but I'm suggesting food that's not only tasteful but healthy. The residents are sedentary thus require fewer carbs. Brown rice vs white rice. The exercise program has not captured my mother's interest. Partly, I believe, because of the few exercises provided, type of exercises and frequency. Upper body strength is one of the major problems my mother has. There are other ways to gain this beyond the ball toss. If more is being done, it's not being shared with me. Perhaps consider grouping residents of similar physical capabilities. If you do, thanks. One major complaint is the lack of a variety of activities. I looked at the activity form and was concerned about the type and number of activities available for residents. There are people who may want to engage in other activities but haven't given any feedback on this. Please ask, if it hasn't occurred, yet. Finally, the buzz from the calling buttons is nerve racking. I'm certain Sylvan has pursued alternatives but that constant noise is grating on people's nerves and could possibly create other behaviors that are not desirable. I want to thank the staff for working with us and with my mother during the transition. It was a little rocky and we understand that the problem isn't always what it appears to be. Gathering all the data and pursuing the full picture of a problem(s) is indeed a virtue when working with a place like yours.

This community was very friendly.

This community was very friendly. This seemed like a place where my mom could stay forever. When I visited, I was not able to be shown specific rooms to see what they were like. I do not remember exactly why we did not choose this community, but overall it was a nice community.

Toured with Allan. Had questions about cable, cancellation policy. Stopped in again on Friday, still no answers. Friend has family member there, noticing some changes recently that were not good. Facility does not appear well kept. 3 rooms avail. Allan called back 9/24 with answers to questions.

Needs improvements to justify cost

My mom said the food is bad. They have NO activities except for bingo sometime. I would say you get what you pay for however they raised my moms fees from [fee removed] to [fee removed] a month and it is not that nice of a facility. They said they did it to everyone and had to to be competitive. That rate is sickening with no activities.

Needs Improvement

They don't do their job like they should. They don't follow through with things. We have had issues with the care services since my love one has moved in. My loved one hates the food. They serve hot dogs, and childlike food. They just started with the activiites. Memory care and asssited living is not separate. This is not a place that I would recommend.

Short Staff Effected Overall Enjoyment

Try someplace else. They were so short staffed, they had care givers and nursing staff cooking in the kitchen. They were over worked. They couldn't provide decent cleaning, or care for their residents. All the foods were frozen foods. They had some activities but they were minimal at best. Nothing my loved one had any interest in. They had high turnover. That created issues with things such as meds.

[Name removed] was very warm and understanding, she would be a fantastic care giver, but the facility itself fell way short. We got a overwhelming "Nursing Home" feel. Rooms were super small, no room for anything but a bed and a chair. Mom and Dad could not stay together in the same room (deal breaker!!!) and therefore cost was high, paying for 2 rooms. Facility smelled like disinfectant and the hallways were narrow and cluttered with dirty linen and housekeeping carts. What they referred to as a "homey" feel, felt more unrefined. Transport services were very limited, only for group outings.

Very Unsatisfactory

This review is for the Chapel Crossings location of Sylvan Crossings. I was looking into moving my other into Sylvan. It would have been ideal for her to live here, as it is quite close by. However, I was far from impressed with them. We initially had them in for two home visits. On the first visit, one of the people in charge came. One the second, someone who was just a general employee came. After those two initial visits, we were never able to get any sort of follow up from Sylvan, no answers to our questions, and no further assistance. It was easy to make the decision to ultimately place my mother elsewhere.

Not what we were looking for...

Our tour of the Sylvan Crossing Hunter Ridge made us reakize we would never put our mother there.The place was not clean and was very opressive, I was there with 2 of my sisters and we walked away thinking we couldnt put our mother there.

My husband and I did not have a good stay at Sylvan!!

The staff very seldom checked on him. The food was horrible. When I asked about living there and if they offered assisted living they said yes. After I was there for a short time I realized the people there had greater needs than myself. There was less socialization and they never addressed my care plan. I would not recommend this community.

The entire care, nursing, and administrative staff saw an ice cream truck, so they all ran outside leaving the residents alone in the building, 10 minutes later they all came back with Ice Cream cones. I couldn't believe my eyes.

Horrible

Clean place but very outdated drug residents up way to much so staff don’t have to deal with them would never recommend this place to anyone worked in health care for over 20 years and this is the worst I have ever

Bad management

Great place back in 2017. Newest Director and Assistant director don't really care how things get done. More people have passed away this summer, and possibly could have been helped if the Director would have given hospice the call sooner. Wausau facility is not a place I would send my family any more.

Very dissatisfied

Very disappointed! The building smelled and the staff smelled of cigarette smoke. I’m okay with tattoos, but this generation they serve is not. Manager seems not up to date on nursing issues and an RN only comes 3 times a week. Food also looked horrible
