About the community

Our Alzheimer's and dementia care community, also known as Clare Bridge, is rooted in €œa person-centered approach€. That means we help our residents find a sense of belonging and purpose while also helping them preserve who they are.

Your loved one will stay engaged with a gentle daily structure of planned activities to help them maintain their abilities and encourage the use of their current skills. Brain games, dancing, cooking classes and music programs are just a few of the ways our residents enjoy their days.

The Clare Bridge Daily Path is an evidence-based approach that offers a gentle daily structure of six planned programs per day, a routine that helps our residents maintain their abilities, encourage the use of their current skills, and offer a sense of security and accomplishment. Working in small cluster groups, each resident is surrounded by others with the same cognitive abilities, to give them a feeling of fitting in.

Staff is friendly and eager to respond to need. Overall the facility is clean. Communication requires improvement. It was frustrating not having one point of contact that has the status of resident, from medication management, daily activity, medical treatments, dietary status, etc. Management tells family members to communicate with the staff, but information is not communicated with shift change. Staffing is not enough to handle contractual terms. Family may get caught up in the middle between Home care agency staff and facility staff as to who performs what tasks, and care may fall through the cracks. Overall, felt like we were oversold on services and coordination they say they provide.

It seems to be working out fine.

It seems to be working out fine. I haven't had any major complaints. It's clean, the staff seems to be friendly, and so far we haven't heard mom tell us anything to the contrary either.

Beware

I would tell friends not to put your mother there. My mother was in the community and was sent to a hospital. Brookdale sent her to another hospital and told me the wrong hospital than they sent her. After finding the correct hospital I found out from the nurse at the hospital that my mother wasn't changed and was raw from this. This made me decide to remove my mother from this community. Her medicines were messed up as well.
