Very poor care. Always short staffed. Unknowledgeable administration regarding the safety and laws regarding an elderly population. Medications are not given on time or missed altogether. Lack of daily activities and outings. Please do your research and visit this facility (unannounced) prior to moving your loved one here. We are moving our loved one to another facility. I don't feel my family member is safe here anymore. I fear the lack of staff will continue to create a very unsafe environment for the residents.

It is an unsafe place to send a relative and the president of the company recommends removing residents if relatives think they are not safe. Picked it based on a recommendation prior to when it changed owners.

Over charging w/threats to send to collections when you loved one has been a resident less than 45 days. Upcharging meals that are given in small styrofoam container. Lack of staff Medicines not available due to poor re-ordering that has occurred several times. Extremely rude argumentative admin. Employees. Promise to work with needs for resident during contract signing and then renege. Lots of issues!!!! Most imp. Activity and much time involved is giving wine out to residents nearly daily. Priorities are odd.

This facility looks great but provides substandard care in the memory care area. This assessment is based upon previous experience with residential memory care. Residents spend the majority of their time in front of a tv. Average staff turnover is every 2 weeks without notification. Most caregiving staff are indifferent to the individual needs of the residents and are often hostile to family members who ask questions. They have no Memory Care Director and no medical personnel on-site overnight to assess any medical issue that may arise. I would not recommend this facility to anyone. I am planning to move my family member as soon as a vacancy comes available at another facility.

My relative started out in assisted living. They did very well enjoyed the residents and the autonomy. They didn't have a lot of things to do but he enjoyed the events they sponsored like picnics and holiday party s. When moved into memory it was awful. Nothing to do. The folks sit idle and stare at each other Just waiting to die. The laundry rarely gets done. They only have one food option. Most lose a lot of weight. New management doesn't care. The corporation that took over ignores our concerns. We're on a wait list to move somewhere else. Don't be fooled by the appearance. Rotten apples look good on the outside too.

My father was in memory care and they were at less than half capacity, so you'd think they could provide decent, responsible care. Within a week his first fall was a mystery. He had serious bruising and a cut wrist, but no one could say exactly when or where or how he got hurt. A few days later, while we were searching for a better option, he fell while standing up in the main living area. He was the only resident there and 2-3 staff members were across the room. Staff watched it happen, and even though he was know to have balance issues and was prone to standing up abruptly, no one thought to position him near a staff. He smashed his head on an end table and died about 10 days later. The facility is beautiful and they tout the dining options. The company culture is just so twisted to place so much emphasis on appearances and to value actual care so little. Just stay away from this place.

Very little that we were told about this place was true. We spent 8 months at Modena. The facility is very nice. The problem is the management. It is understaffed and the staff is poorly trained. Often waited an hour to get a meal. The general manager does not like to be questioned and will retaliate if criticized. She lost half her office staff in the first 6 months and the caregiver staff is like a revolving door. An LPN is in charge of care and medication. The inconsistencies in how this was handled is the major reason we left.

They absolutely have amazing move in discounts—because their so-called nurse(who is no more than an LPN) and executive team refuse admission to anyone who actually needs help! As a result, this place is empty. When it became time to evaluate my parent, the nurse refused to see him, she refused to speak to me, and she refused to contact a nurse or a Dr. who has his case history. No one with any authority would even agree to a conversation with me. I spoke to their moving partner, Mike Gallagher, and he said this happens to alot of people! They lied and said that the state had regulations prohibiting residents with wounds (my parent had no wound, he had a skin graft). I called the state, this statement about wounds was a blatant LIE-the only thing the state worries about are pressure sores and he has none! Please note, they had my money and still refused to do their due diligence. A total of THREE other facilities have accepted him. Steer clear of this place!

Staff turnover is a problem. They are disorganized and poor customer service. You can ask for specific help with something and not get it. They don't clean the residents rooms very well and lose the residents personal items, like clothes and towels. People are poorly trained and so they can't keep anyone very long. Incompetent employees.
