My brother has been a resident at the Reserve of Oswego for a little over a month. My experience has been very favorable during his stay. I think he is treated very well. My brother tells me the meals are tasty and nutritious. They keep the residents very busy with games, crafts and visiting with each other. I also like that family members are welcome to take part in the activities.

After moving my father from one rehabilitation center to the Reserve we learned that all Nursing Homes aren’t the same. My family is relieved at the attention my father is getting and how they keep the patients active. The professional care he is receiving now seems like family that really cares about him. Thank you to all of the people at the Reserve for making him feel welcome.

While her stay was not the longest, it was the right place for my friend in her final days. When my friend moved to The Reserve the staff and residents made her feel very welcomed. The activities that are conducted are all meaningful, fun and important for the residents. Meals are well planned out and tasty. The entire facility is spotless and very well kept. The staff, from the CNA team, to the nursing staff to the kitchen folks and on yo the management team are all top notch, professional, compassionate and well prepared. I highly recommend The Reserve of Oswego for your consideration for your loved ones care.

My family members lived here for a short time. Lack of staff Used agency that did not care. Plus lack of management. Had spoken to upper management and never did anything. The director is useless and labeled my family member as psychotic!!!! This company only cares about money! My family members passed away and no condolences! If you value care for your loved one do not move them here. This place does not respect their staff or the family of the people that live here.

We take pride in being voted "Best Place to Work" by our staff in 2023. Our last visit from Illinois Department of Public Health showed us in complete compliance with all licensure and standards. It is a priority that each resident's rights are upheld by our staff and management. Dignity and respect is always practiced within this community.
They have been very open to our feedback. We picked this community, because it's really close to my Mom's house so she can visit Dad often, small size of facility, they listened to our concerns/fears and shared their personal experiences with memory loss caregiving, A Place for Mom got them to work with us,to negotiate on the costs.

They make sure all residents are involved in activities, no matter their status of care. Family is always welcome to come and join in the daily routines. They make sure holidays are celebrated and that family can be invited to join the festivities. Looking forward to their Special Christmas dinner coming up soon. They could use more staff so other workers could have a day off. They do the best they can with the workers they have.

The the stafff is very friendly. They care for your loved one. They recognize me by name. They are always open to suggestions. They sometimes have a shortage of staff. If they were properly staffed, they would have a higher rating.

My husband was there for 6 weeks and we had to move him. His basic needs were not met, even though they promised they would be. He has dementia and does not have the ability to take care of himself. For the 6 weeks, he wasn't shaved. His hands were dirty and he didn't even have a wash cloth in the bathroom. He had on women's shirts a couple of times. He has aggression issues and they didn't know how to handle it. He's been gone a month now and I still don't have a corrected invoice for two months ago.

Out of respect for your loved one's privacy and dignity, I can not comment or explain the specifics of their routine on a public forum like this, but I hope you will return my email or come to the community and we can discuss their situation. Our staff responded to the changing needs of your loved one, and as you've stated, aggression issues add another layer to a complicated disease process. We empathize with your situation, but we also need to ensure the safety of ALL residents in our care. I do hope you will respond to my emails and set a time to meet with me. My best,
I would say to make sure that what they say they are going to do that they really do it because that was the problem for us. There are activities but my loved one does not do these activities in the community. The staff are okay and there is a lot of turnover. The communication is very slow unless I am initiating the conversation instead of being proactive.

The manager, [name removed], is the best and she does it for the entire community. The nurses don't seem to have much patience for the residents. My loved one is safe. There are activities for the residents to participate in.

Since we moved in 5 months ago the facility has declined greatly. Several of the administrative staff have been replaced . The maintenance and housekeeper have both quit, therefore the facility has become unclean and the rooms are not being cleaned properly. We were promised a shower three times a week but if we are lucky once a week -if we complain. The caregivers are not answering the call lights in a prompt manner in the night. I don't think there is a good training for them to attend to residents with dementia. The caregivers are expected to do more than caring for the residents, like house keeping, feeding, cleaning rooms laundry, etc. The meals are always late and the times have been adjusted to accommodate the schedules of the caregivers.

Autumn Leaves/Oswego is a wonderful memory care facility I visited about 10 facilities and immediately noticed the difference in the way the residents were treated with love and care at this home. They are treated like family. I highly recommend that you pay them a visit.

Wonderful facility! My dad has been there about a year now. Staff takes great care of him. They are always attentive, caring! Professional. The administrative staff always has time for family concerns, especially [name removed] & [name removed]. [name removed] does a great job & activities and engaging residents. We can't say enough about the loving care all the staff at Autumn Leaves gives to our mom. The atmosphere at Autumn Leaves is very family like. Everyone looks out for reach other. The residence is very bright and cheerful and the staff work hard at keeping the residents engaged with many acitivities we are glad our mom is being taken care of so well when we are not here.

My husband attends the Day Stay and loves it. He loves all of the staff and is excited to see them each week. They are very caring and compassionate and make him feel welcome each time he arrives. I see him happy, is busy and sings and talks and laughs with residents.

The care at this community was not what we wanted. The staff wanted my father's door closed at all times. The staff constantly called my sister for no reasons. The staff called an ambulance that was not needed. We were informed that this community calls the paramedics when they dont want to help residents. This community is not recommended due to the care. My father is no longer there and doing well. Families should tour this community before they make a decision for the care of the family.

I toured this community. And it was like a home feeling. The staff was caring. They seemed to have a lot of nice activities for the residents. The menu looked appealing. We would recommend this community to other for care and value.
