Since she has been at H2H, their team and the physician that comes to the home have better managed her dementia personality changes. H2H has 24/7 caregivers at the home at all times. When I visit, I get to know the caregivers and observe their interaction and hands-on support they provide to the residents. They treat the residents with kindness and respect. The Home Manager communicates with me, which is super important to me.\nH2H makes home cooked meals, the residents clothes and bedding are washed, medications are dispensed, they have a calendar of daily activities in which they try to get the resident's engaged and involved. Sumpter Senior Living is located on 1.5 acres, directly across the street from the Sumpter Senior Center and Sumpter Police Station. The facility is 4200 sq. ft. with 10 bedrooms, 4 bathrooms, and an outdoor pavilion.<br><br>PROGRAM GOALS<br><br>The overall goal of Heart to Home is to provide safe and quality personal care, supervision, and protection while maintaining dignity and respect of each and every resident in our care. <br><br>Specific program goals include:<br><br>Assisting residents in personal care activities such as dressing, grooming, and bathing. <br>Assisting residents in appropriate socialization, including productive and independent leisure time, one-on-one interaction, and group activities. <br>Assisting residents in understanding and expressing their emotions and feelings, focusing on building good self-esteem and a sense of self-worth. <br>Encouraging and helping residents in maintaining involvement with family members and other significant people in their lives. <br>Accurately assessing and responding to the unique and changing needs of each resident.
