The Clare Bridge Daily Path is an evidence-based approach that offers a gentle daily structure of six planned programs per day, our routine that helps your loved one maintain his or her abilities encourages the use of current skills and offers a sense of security and accomplishment. Along with our programming, you’ll have peace of mind knowing that trained staff is on-site 24 hours a day, seven days a week.
We are committed to building person-centered partnerships with relationship-rich care. In our care model, we build relationships with trust, loving concern and a sense of family. We listen to your loved one as we problem-solve behavioral expressions that may arise. Knowing and being present in relationships with your loved one gives us a strong foundation to anticipate any unmet needs and lessen behavioral symptoms.
Our apartments can be personalized with familiar furniture and belongings so your loved one will feel secure and comfortable. Our apartments have plenty of natural window light and some even have a garden view. You and your family can choose from several floor plan options including a private studio suite or a companion studio apartment, and each apartment has a private bathroom.
And we use mealtime as an opportunity to offer healthy menu choices for your loved one and create a sense of belonging. Your loved one will enjoy dementia-friendly menus with familiar and easy-to-manage selections, small dining rooms with consistent pre-selected seating and personal dining assistance.
Your loved one can enjoy sunny days outside in our large courtyard with walking paths or meeting up with friends in the game room to work on a puzzle. We also have areas of the community that are set up for safe engagement in gardening, kitchen work, handyman help like a woodworking station, and other household and normal daily tasks. Life skills activities provide a sense of purpose, meaning and belonging as residents complete tasks related to everyday life.
