We are having a VERY hard time communicating about Mom's pills. She may be forgetful, but she knows her pills. We have requested them to be given at breakfast and dinner MANY times and they still are not. Also, she is not getting them all. What else do we have to do? If they are not given properly tomorrow (Sat.) and Sun. we will call whomever is above the director at Saber in Green Ridge. We thought we were getting good care as far as everything is concerned. We also signed a blank care plan which we were told we would get before we left for FL. Did not get this. Please advise - are thinking this was not the Assisted Living to place Mom in.

I would not recommend it unless they had no other choice. Especially if the resident was mobile and was functioning cognitively.

I was deceived believing that I would have physical and occupational therapy in a gym equal or better than the rehab facility that I was in. I was also told that I would have transportation to and from my doctor appointments in Chapel Hill. Neither were true. Therefore. I had to find outside services and pay [fee removed] one way to my doctor visits. I left because for me I did not see the value for what I was paying. Besides meals were barely decent or nutritional for diabetic. It was an overpriced hotel with assistance. The Executive Director is aware how disappointed I was directly and through her staff. Although she never addressed how my stay was going. There were several staff members that respected and truly cared for the residents. Not many residents were satisfied with the food or accommodations with there needing repairs for several bathrooms. Maybe for the resident population the OT & PT was adequate , however for a higher functioning person they will need to have to get outside therapy. It's hard to say if I would or wouldn't recommend. I know definitely I would not recommend to those needing memory care to become a resident at Meadowview. They do not have the staff to take care of them or the accommodations.

It smells bad, some of the staff are really kind- especially the kitchen people. Nurses are aloof, takes more than an hour to get meds. Director is never in the building. Nothing is done proactively. Activities are minimal.

Do not let the nice looking pictures fool you, this place has major problems! There is currently a Class Action Lawsuit against against Gabriel Manor and other Saber Health facilities. They are on their fourth executive director in two years, staff turnover overall is ridiculously high, complaint by loved one that CNA spoke to her in a demeaning way, medication errors and failure to give meds, failure to notify family when loved one is sick or seen by the "doctor", BM odors in the hallway, inconsistent with ADL's, food and liquid spills on dining room floor two hours after a meal. Meals were great when there was an actual chef but even this has gone downhill in recent months. I have personally seen all this and more!

Where to begin...I'm a family member. I'll start with my initial reactions. The grounds are awful, messy and no place to relax in this nice weather other than a couple of seats in front and they are always filled. The patio areas seem to me like big cages with broken furniture. Not sure what happened with those. Inside, it's a clean, newer building. The front desk woman, Bernece, is lovely and very helpful since day one. The home is not very creative in it's design choices inside but still has that new feel. No obvious smells in the hallways, no signs of issues at a glance, it seems clean. Having talked with some of the other people who live there on my visits, they seem fairly pleased with the physical living arrangements as they are. Spent a little more time there and spoke to those residents and some issues became a little more apparent. I'm semiretired so I make some visits, usually in the morning, but also have visited during the evening a few times. I will add that I'm retired from health care so I know a little bit about regulations. Staff and staffing seem not so good (actually in all honesty, it seems bad). I heard someone yell and saw someone sitting on the floor in their room one day. I was the one who went to find a staff member and it took forever and apparently there is no nurse in the building. No one is even in the hall at night and a couple of lights were on for a very long time several times and staff attitudes seem very cold. I've heard them complaining in the hallway. My family member told me they complained to the administrator of the place and his attitude was "even worse". I've actually witnessed him admonishing loudly someone in the hall. I won't go into it here but it was mean and very inappropriate for me to hear. I hope that doesn't happen often. I do know about HIPPA and common respect. Other residents nearby during a recent visit heartily told me that nothing they have asked for has gotten done. "we asked him at a meeting but nothing ever changes" "he doesn't even talk to us""what are you going to do, other places in the area are worse" "the food is terrible but they don't care, they're cheap" "I don't want to complain, I'll get in trouble with him" I know they seem to have a big turnover because everyone is different often. It seems to be getting worse there instead of better. I don't make the decisions for my family member but I know it's expensive. I suggested a move at one point but they don't want to and that part isn't my business.

This started out as a great place for mom when they first opened. Four years later, it's clear huge cuts have been made to staff, care, and nutritious meals. What's still good? The staff members are kind and caring. The facility itself is nice and spacious. Nothing special, but adequate. What's bad? The facility is so understaffed that residents sit in their own waste for hours before being helped. Residents who need to see a doctor are not provided any assistance getting to that doctor - they must arrange their own transportation. If they choose to see Dr. [Removed] (who is listed as the doctor for this and many other facilities with hundreds of patients), the wait is weeks long. When a resident is going through the transition from ambulatory to needing a wheelchair, for example, that three weeks is translated into multiple falls requiring hospitalization simply because a wheelchair cannot be approved by Dr.[Removed]. Unacceptable. If you send a family member here, be sure you can afford to hire a private nurse or a care assistant to come in regularly to check on your loved one, or be there daily to assist, as I see some families do. I'm also deeply concerned about the nutritional value of the food. We absorb fewer nutrients as we age - fresh, wholesome food becomes even more essential. High sodium, empty calorie food like hot dogs, mac and cheese and pasta simply do not provide the nutrients residents need. There is a salad offered - but iceburg lettuce and tasteless tomato offer little nutritional value. I doubt this facility would pass a thorough inspection - especially in light of the patient/care giver ratio. It's a highly profitable mechanism for the owner company, however. My mom fell multiple times and was sent to the hospital each time - nobody was aware that she was dehydrated, malnourished and needed extra care. SO disappointed in the Gardens - we were big fans when it first opened.

After five months at FM, my Dad died. After many needless trips to the hospital, there was no peaceful passing for him as there could have been if Hospice had been allowed to engage fully. I write this review one month after my Dad passed. As of today, [name removed], the Executive Director, has yet to reach out to me or my mother to share her condolences about the loss of one of her residents. Absolutely zero compassion. My dad was a veteran, a husband, a father, and a grandfather, but to FM, I believe he was just a monthly credit card payment. As I wrote my Dad’s obituary, all I could think about was how I let him down by choosing this place. I can’t turn back the hands of time, but I can write an honest review of Franklin Manor so you can make an informed decision about where to place your loved one. Choose wisely. If it were possible to leave negative starts I would. 1. Ask them this, "Do you ever have an actual nurse on duty, ever????" The answer is NO. Only CNAs and Med Techs are supposed to care for people with advanced dementia. Even the head patient care coordinator isn't a nurse! A doctor is never on-site. A PA rounds at the facility once a week (and is on call). 2. The staff tell you that they don't have enough help or training to "really" meet the needs of the residents that live here 2. In the last year, they have gone through three Executive Directors, three business office managers, and three patient care coordinators. (BTW, none of them nurses, PAs, or MDs). My best friend's dad went there, and within five months, he had a wound on his bottom so nasty that they made him leave because they couldn't handle it (stage 3). It's crazy because they caused it by not moving or changing him. He had no wound when he first arrived. BEWARE THIS FACILITY!!!!

STAY AWAY. Do not put your loved one in their short term rehabilitation. The PT staff were excellent and caring but CNA severely understaffed and low quality skilled nursing staff who can not catch or address health issues. My mother entered stable and 10 days later left in an ambulance with systolic BP over 200, on oxygen and hospital reported her COVID positive. She waiting 1 to 2 hours for help to use the bathroom often having to go before they came. Read other reviews carefully, they are not from patients or caregivers but from staff and family of staff that work there. STAY AWAY

Please don't send your love ones here, warning , one of the worse places in Cleveland for a person to have to live. Someone very dear to us was allowed to be alone unsupervised at night outside our loved one ended up on fire and burned , this is Real how could this happen to another human being, did anyone from this Nursing home care a little.How can you allow this to happen. Shame on you all. God sees.

We went for a tour at this community and they had us just sitting there and we could not get in. The manager on duty was not responding and told us to wait. we waited for over 20 mins and seemed like they were just not getting to us. She seemed to be to busy and if that was the first impression we did not want to run into another situation where they were to busy to help my loved one. No one else came to help her out and we just knew it was not the right place.

This facility at Gabriel Manor offers some of the poorest care that I can imagine is available in the area. I believe fast food restaurants have less employee turnover. The lack of professional upper management that is aware of what is happening in the facility is unbelievable. You can go to the facility almost anytime Monday through Friday and if the director happens to be there you will see that she is locked up in her office and rarely ever leave it. For the most part I believe the employees truly care but without Direction proper training an experience you cannot expect any more from them than what they are providing. At best most days Gabriel Manor I believe runs on autopilot. Their method of dealing with Dementia patients is to drug them up with medication so that they don't have to cope with them or just put them in the room and shut the door so that they do not have to listen to them. On various occasions I have witnessed pills on the floor that patients are not taken that has been given to them without someone verifying that they have actually swallow the pills. Most of the time the facility is kept very cold to the comfort of the employees and the patients are usually complaining about how cold they are. Do your loved one as well as yourself and your family a favor by pass this place as if it has the plague and find someplace else to look at.

I can assure you these ratings are from people inside this facility. If you love and care about your loved one do not place them here. This place is not what it used to be. It smells, staff turnover is tremendous, and whomever said they had a family member fall and it was never reported; BELIEVE IT!! I have encountered the same thing. The new director will spin you like a top. You can never get a return phone call, most of the time they don't answer when you call. I am warning you do not place a family member that you love and care about in this facility. I have made several request to have them adheared to a few days and then back to normal if not worse, in the next few. Forget that we are changing the guard and it will get better....it WON'T. I am in the process of finding a new facility as we speak. This place is to expensive for the quality of service it provides.

Biggest scam to Seniors. High rents with levels of care : medication fees with one full month's rent due prior to admission that is non-refundable. Poor nursing care; short staffed. Place should be shut down by the State.

Avoid this place at all cost!!! If you open the closet, all the skeleton's will fall out. I would not recommend this place for your loved one. Just poor care and tired staff. Staff is so short, they are working the heck out of what they have just to have coverage. If you don't want your loved one to go down hill quickly, do not put them there.
