Medicaid

Medicaid is a federal program that is administered through the states. Virginia’s Medicaid program is called Cardinal Care, run by Virginia’s Department of Medical Assistance Services.

Medicaid basics

If your loved one is eligible for Medicaid benefits, it’s important to know what Medicaid does and doesn’t cover depending on the facility you’re looking for. For a skilled nursing home, Medicaid will cover custodial costs such as room and board. Medicaid will not cover room and board for an assisted living or memory care facility but it can cover in-home medically-required health care needs, as in-home services can happen wherever the recipient is living.

To know if your loved one qualifies for Medicaid, you need to first apply for Cardinal Care. You can also apply over the phone by calling 1-833-5CALLVA, or you can drop off or mail a printed application to the Fredericksburg Department of Social Services. Programs that can help

If your loved one does qualify for Medicaid and you are looking at an assisted living or memory care facility, programs that can help offset costs include the Commonwealth Coordinated Care Plus waiver (CCC Plus), a long-term services and support Medicaid program for individuals over 65 and/or who have complex care needs. Designed to help individuals stay in their homes as long as possible, qualified applicants receive a personalized care plan.

Another benefit is the home and community based services (HCBS) waiver program for people on Medicaid who require in-home health care. A third waiver program is Developmental Disability waivers for people with a developmental disability. It’s possible to qualify for more than one.

Medicare

Those over 65 will be familiar with Medicare, the federal health insurance program that is primarily for seniors. Medicare can cover skilled nursing care but only if an individual meets certain requirements. These include but are not limited to:

  • The senior must have Medicare Part A, also known as hospital insurance.
  • The individual needs to have been in the hospital for a minimum of three days in a row and must still require medical care following the hospital stay.
  • A doctor must recommend skilled services for the individual, such as nursing care or therapy.

The coverage will still only be on a short-term basis. Medicare doesn’t cover nursing homes as a long-term care option. You can see the full list of requirements and more about what Medicare covers for skilled nursing care on the Office of the State Long-Term Care Ombudsman website.

Assisted living and memory care

If you are interested in an assisted living or memory care facility for your loved one, Medicare will not cover custodial costs, like room and board. However, Medicare can help offset costs by covering medically required in-home health care and other health services such as prescriptions, medical equipment, and some preventative care services.

If you’re concerned about costs, there are Medicare Savings Programs available for those who qualify. The purpose of the programs is to help low-income individuals with costs like Medicare premiums, deductibles, and copays by reducing them or fully covering them.

Applying for Medicare and resources for help

For seniors who need to sign up for Medicare, they can do so online or in person at the Fredericksburg Social Security office. Please be aware that the initial enrollment period for enrollees is three months before a senior’s 65th birthday.

Understand Virginia laws and regulations for nursing homes

Laws and regulations for senior living facilities depend on the type of care being offered, whether it’s a nursing home, assisted living community, or a memory care facility.

Nursing homes in Virginia

All Virginia skilled nursing homes are licensed and inspected by the Division of Long-Term Care Services under the Virginia Department of Health. Regular inspections are carried out to ensure facilities are compliant with state and federal laws and Medicare and Medicaid requirements, and facilities can also be inspected following a complaint. All inspection reports can be found online through the state’s Provider Search System. In addition to being licensed and adhering to regular inspections, skilled nursing homes in Virginia must meet certain standards, such as training requirements for nurses and activities on-site for residents.

Assisted living and memory care in Virginia

Since memory care facilities are specialized forms of assisted living, they are seen as the same under the law in Virginia and therefore follow the same laws and regulations. It is important to note that 100% of our partners in Fredericksburg do offer some aspect of memory care. Both assisted living and memory care facilities must be licensed by the Department of Social Services. The same department also carries out inspections that happen annually to ensure the facilities are compliant with regulations in regard to safety and sanitation.   • Facilities must help residents with general daily tasks as needed, such as grooming, eating, and dressing.
  • Facilities must have medication management plans in writing and medication must be administered by a licensed health care professional or an aide who has completed required training programs. The aide must also be registered with the Virginia Board of Nursing.
  • An administrator is required at each facility to manage the day-to-day operations and overall governance. There also must be adequate staff who know first aid and there must be enough team members to manage an emergency.
  • Every facility must have a full-time health care professional on-site or a contracted health care professional who visits at least every six months for oversight.
  • Facilities that serve residents with dementia, such as memory care facilities, must follow staffing requirements and make sure their location is secure to prevent residents from unsafe wandering. Windows must have devices to keep residents from opening them, and exterior doors must be secured and monitored.
