AND SKILLED NURSING
When assistance is needed only for a short time,
such as rehabilitation after surgery or illness,
Hickory House Nursing Home offers advanced
rehabilitation services. If 24-hour skilled nursing
care is required, our long-term care facility’s
nursing team will provide care under the
direction of the patient’s attending physician.
Heatherwood Retirement residents can take
advantage of having temporary full-time
medical care or rehabilitation services on the

your apartment when you return home.
WHAT IS A CONTINUUM-OF-CARE CAMPUS?
We understand that at different stages
in life our residents may need varying
levels of care. That‘s why we‘ve
created a continuum-of-care campus.
It‘s a place where our residents can
live comfortably in our independent
living apartments or can receive shortterm
rehabilitation services in our
skilled care facility – and a variety of
services in between.
This environment is designed to
provide residents with a home that
changes and adapts just as they do.
That way, they can stay in one place
where they are comfortable while they
receive the specific care they need.
