Commonwealth’s Sweet MemoriesTM memory care program has been created to deliver the most exceptional care and lifestyle to our residents living with Alzheimer’s disease and other forms of dementia.

From the welcoming, homey feeling of our memory care neighborhoods to the experience of our management team, everything in our program has been carefully chosen to deliver the best memory care experience possible.

Understanding that each person in our care has different needs, Sweet MemoriesTM takes a resident-centered approach. By developing a personalized care plan, we can provide support that not only meets needs, but also promotes abilities and independence.

Program Components

Sweet MemoriesTM consists of three primary components to ensure an exceptional memory care experience.

  1. Move-in: From physical to emotional needs, we guide our residents through every step of the transition to our community, whether from home or hospital.
  2. Evaluation through life history: We get to know each resident personally. We conduct a comprehensive interview with family members. We talk. We ask questions. We listen. We get to know our residents as people. We learn about their medical, mental, emotional, spiritual and intellectual needs, as well as their habits and preferences.
  3. Resident-centered care plan: With this very personal knowledge, our leadership team creates a customized care plan to meet the specific needs of the resident.

Signature Programs • Signature Experience

Unique and effective, our signature Sweet MemoriesTM programs make a tremendous difference in the lives of our residents and their families.

One Sweet Memory: Winner of the Best Practice Award from the Virginia Assisted Living Association (VALA), for going above and beyond the standards of care. Our team members regularly capture highlights of each resident’s day in photographs that are shared regularly with family members.

Expressions: When verbal skills begin to fail, art can become a means of self-expression for residents. Conversational prompting stimulates memories which residents can interpret using a variety of mediums, including painting.

Special Touch: Designed specifically for those with late-stage memory loss, this program provides “special touches” that make a difference in care.   • Few to zero activities. Filthy rooms and bathrooms.

    Four directors in 20 months. Poor communication with families. One bingo game all summer. This was the only activity. When there were activities it was bingo everyday. Residents have rarely seen the activities director and for several months they did not know who she was. Room and bathroom not cleaned in at least a month. Toilet filthy week after week. Replaced carpet. New carpet too plush for wheel chair. Transitioning from carpet to floor and vice versa is extremely difficult with wheel chair and a trip hazard with walker.

    Moved my elderly relative into Greenfield of Stafford (They have various ones in the area) at the beginning of JAN2019. When I initially looked at it, I went to it unannounced and received a tour, etc. It is a clean, fresh smelling facility. In other words it doesn't smell of urine or "old people". You can either have individual rooms or shared rooms; individual rooms costing more. From your base cost (being the type of room, individual or shared), the additional cost is the level of care that will need to be provided. It is a 2 level facility, the top floor being for the memory loss. There is a security elevator so people can not wonder off. Cable TV is included (basic Comcast cable), phone line is at individual cost if they want a phone in their room. Staff is attentive and friendly from what I can concern; my relative and other residents I have befriended have told me they are treated well and not mistreated in anyway. I have asked staff members if Greenfield treats them right as employees and have gotten the same responses. They have a monthly calendar of events posted and distributed to all. Security for elderly that might wander off or go outside alone and are prohibited from doing so works as well. I experience seeing an elder resident trying to go outside, and the doors immediately locked and no one could get out? We were not aware at the time why we could not get out... A staff members came and took the resident away from the door, and the doors then unlocked for us. They have some type of security that prevented her from opening the door. It was comforting to see that that type of security was in place, had my relative needed it. All and all this facility as of FEB2019 is excellent and I have nothing negative to say. I will revisit the review if I see something I am not satisfied with.
