Heart to Home has two facilities that are owned and operated by two Registered Nurses, each with 17 years of experience.<br><br>Heart to Home's Wendy Manor is a licensed Adult Foster Care Home, providing 24/7 personal care, protection, and supervision for up to six (6) individuals who are aged and/or diagnosed with Alzheimer's/dementia. Wendy Manor is a ranch style home with 2 private bedrooms, and 2 semi-private bedrooms, bathroom, kitchen, dining room, living room and laundry facilities on the first floor. French doors lead to the beautiful backyard with deck and handicap ramp that provides a safe, private, and comfortable living environment. Wendy Manor is located in a quiet subdivision near Pennsylvania &amp; Inkster Roads. Our quaint home is perfect for individuals with impaired memory as the home layout is easy to navigate.<br><br>PROGRAM GOALS<br><br>The overall goal of Heart to Home is to provide safe and quality personal care, supervision, and protection while maintaining dignity and respect of each and every resident in our care. <br><br>Specific program goals include:<br><br>Assisting residents in personal care activities such as dressing, grooming, and bathing. <br>Assisting residents in appropriate socialization, including productive and independent leisure time, one-on-one interaction, and group activities. <br>Assisting residents in understanding and expressing their emotions and feelings, focusing on building good self-esteem and a sense of self-worth. <br>Encouraging and helping residents in maintaining involvement with family members and other significant people in their lives. <br>Accurately assessing and responding to the unique and changing needs of each resident.
