Seasons Alzheimers Care and Assisted Living

Season's Alzheimer's and Assisted Care is a residential care home for seniors that is located in the city of San Antonio, Texas which is in Bexar County with small portions extending into Medina and Comal counties. San Antonio is a city with a great deal of history. The Alamo is a national monument and historic destination for residents and visitors alike. The River Walk is another popular site as this scenic route winds through the city center and allows residents to peruse shops, dine in delicious restaurants and enjoy the scenery.

The amazing location of Season's Alzheimer's and Assisted Care is not the only thing it has going for it. This group home setting also has a set of staff members and caregivers that are extremely dedicated and compassionate. I wish I could give 0 stars for this place, do not send your loved one here if you care about them at all. My loved one died and no one noticed until the morning. Hours and hours left dead. I thought they had to check the residents every few hours. How does someone go dead unnoticed if they are checking like they are supposed to. They tried to cover it up my loved one's death and the cops had to come. Then all of the staff left the building when the cops arrived because they are undocumented and there was no staff to care for any of the residents. The residents were abandoned. The owner is going to comment on this and say there is not any one with my name with a loved one as a resident and that this did not happen and that this is a mistake. It is all a lie. Do not believe the lies she is going to tell you. I do not understand how this place is not shut down. I did file a complaint. This place is a joke all they want is your money for all her businesses. Do your research. I wish we did

Regarding state licensing, our facility strictly adheres to Texas guidelines. We conduct thorough background and immigration checks for all staff members. Making assumptions about someone's legal status based on language is unacceptable, and we take diversity and inclusivity seriously. 2. In the event of a death, especially if not on hospice, we promptly notify the medical examiner's office and the state licensing authority, as per Texas regulations. 3. Grateful for Seasons
My mother is a resident at Seasons Nursing Memory Care, Nacogdoches Road. The facility is beautiful, but what makes the experience more special is the personal care and attention my mother receives from the staff.[names removed] and the rest of the staff attend to my mothers needs in a warm, respectful, caring manner. [name removed] and [name removed], along with the rest of the hospice team has been amazing, as well. Lenora attends to daily activities to stimulate my mother and the rest of the residents. The care and experience at Seasons Nursing has exceeded our expectations.

Very Pleased - Keeping my Mom active
Various activities for the residents: 1. My mother enjoys the gentleman who sings and plays the piano weekly. She can even request one of her favorite songs. 2. There are dancers who encourage the residents to dance or sit and march with their feet while they shake tambourines and maracas to various types of music. 3. Mom looks forward to the art and craft activities. She has been very proud to show them to family. I am very pleased the Caregivers took the time to get to actually get to know my Mom and her personally.

I would highly recommend it. We had a great experience with Seasons. In fact, I was so appreciative of their care of my dad, I made them 20+ fidget quilts. They treated my father who had severe dementia with so much compassion and they were very supportive and caring to my mother. We were impressed with the variety and number of activities they had for the residents. The staff were attentive and respectful to my dad even when he was being difficult. The staff were with him and cared for him compassionately as he died. We would highly recommend Seasons.

keep in mind my mother is on hospice and receives additional care paid for by her insurance. The reason the cost is high is the high staff to client ratio which this facility provides. In addition the staff really do care about their clients! I would like to see them provide more water to the clients at times other than just meal or snack time.

At 1st it was good and then turned all wrong
My mother was getting excellent care in the beginning(2019). After almost a year and before Covid the quality of care declined. My mother was not getting needed meds and care. Her walker has disappeared 3 times now. I have had to contact the owner regarding issues on average every 6 months. The newest problem delayed feeding of residents. The other problems still exist and the owner always says she is aware, but clearly addressing and fixing the problems is not a priority. The monthly fee is average and was ok as long as the care was being provided. Now that the care is not being provided because they are obviously short staffed, I wonder if my mother and other residents are safe. I am no longer pleased with the care provided..my mother's fall risk has increased as a cane is not safe, but her walker has disappeared again and staff, mgmt and owner don't seems too concerned. Keep looking for a good place for your loved ones. This is not the best place for them.

My loved one was in this home for far too long before I became aware of the numerous problems and violations there. Dehydration of residents was a huge issue. People were kept in Geri chairs all day. Beware of this facility.

I am so sorry, I have been open for over 5 years now and I have had no move outs for the last few years. We allow our residents to age in place, we have a nurse full time, highest staffing ratio and a wait list to move in. We do have few residents in Geri chair as these chairs are more comfortable and we can recline them. If we have them sitting in a wheelchair too long, it is very uncomfortable, and if they start leaning forward the possibility of falling out of the chair. We use Geri chair, Broda Chair with consent from the family and doctor. Please check my Texas DADS website we have no violations regarding our care in the last five years. This is a public website and the list all violations on it. Dehydration - is a very common problem in dementia and Alzheimer's patients - we have hydration stations, every 2 hours we have staff giving water and small snacks as many residents don't eat full meals at one time. We are only 16 beds, 4 staff on the floor ( not in office or management), a full-time cook, a activities director, Lvn & management. No facility in the city or state has this kind of staffing ratio. We have a Geriatric specialist Doctor who comes as needed basis, we have our own lab set up, xray setup, dentist podiatrist etc. We offer hospice, home health - whatever the resident needs. Going back to dehydration - because of the disease process & age many residents refuse to drink water, and as the decline, this is a common problem but we push fluids all day long. Me as a owner round on my residents daily along with my executive director. I am truly sorry that you feel this way but I have had no family approach me with this issue recently, we are very proactive and if see any change we are on it. Sometimes we are too proactive - please call me. If your loved one is with me you have my cell phone. I would love to address any concerns you have.

After dealing with three other senior facilities, it was a blessing when I moved my aunt to the Seasons. These folks are head and shoulders above most other like facilities in San Antonio. Wonderful Place
My Mother has been a resident at Old Tezel location for almost a year. I was at my wits end because she had lived independently up to the age of 92 years and was insistent (and resistant) that she should continue to do so. Mona T was recommended to me and I can't say enough good words about her approach to handling residents. She also imparts this to her Staff. The aide to patient ratio is wonderful and they are very caring. All of this is so important to me because neither I nor my son live in the area so we must trust the Staff with total care of my Mother. From the beginning I have had no qualms or concerns about the care given. The Staff is very conscientious about keeping us aware of any little situation that might be of concern as well as sending pictures from time to time. They also give updates regularly on the positive aspects and their activities. If anyone is considering Season's, please visit and you will get the good feelings I did the first time and also honest evaluation on how your loved one will be cared for from Mona.

Mom is 87 yrs. old and has been living at Seasons for seven months. The Seasons is spacious, new, and welcoming. The food is like Mom's home cooking with a healthy variety that appeals to everyone's appetite. The staff is caring and attentive with the emphasis on personalized care that dignifies those who need help at this difficult time in their life. A Great Place For My Dad
The Alzheimer’s journey with a loved one is not an easy road. Each and every day is a new day for someone who is suffering from Alzheimer’s disease. This disease does not discriminate and no two patients are alike. I thank God for people like Mona [removed] and her staff at the Seasons Alzheimer’s & Dementia Care Facility on Old Tezel Road in San Antonio, Texas. I moved my dad from another facility in Texas, when I was told that they could no longer handle his behavior. It was a large Alzheimer’s facility. If they couldn’t take care of my dad, who would. Talk about feeling helpless and alone! My husband visited several facilities in San Antonio. It was Mona and the Seasons environment that sold him on a place for dad. My dad joined the Seasons family in August 2015. I felt obligated to let Mona know that my dad was in the stage of Alzheimer’s disease where he would become easily agitated and loud outbursts were the norm. Mona reassured me they handle all stages of the disease. Mona and her most compassionate staff welcomed my dad with open arms. Their actions speak volumes! Compassion, hope, love, dignity, and respect are just a few of the traits that Mona and her staff exhibit. I feel that dad is at home. He enjoys the wholesome home cooked meals and the interaction with staff. Seasons provides a clean, safe and secure home-like environment. Seasons is a small 16 bed unit, but excels largely in all aspects of care. It is because of Mona and her staff, that my dad continues to thrive. I know he never feels alone in his journey. I would highly recommend Seasons Alzheimer’s Care Facility. They provide the peace that I need, knowing that my dad is being well taken care of and his needs are being met. Also, kudos to Heart To Heart Hospice for the care and kindness they demonstrate towards my dad.

Since my Mother's diagnosis, I have lived by one rule: Those who are forgetting should not be forgotten. And so the journey began of finding a "home" for my Mom where she would be cared for with dignity, compassion and, with hope, a lot of love. After extensively researching options, I chose Seasons. This team, led by owner Mona Talukdar, is dedicated and fiercely devoted to the care of Alzheimer's patients in a most holistic manner. Their clinical needs not withstanding, Seasons offers care that is also spiritual and emotionally healing. From art and music therapy to the regular animal visits to walks on their secure property, private rooms and freshly prepared meals...every step, every aspect of care at Seasons is focused on optimizing the well being and happiness of the Alzheimer's patient. The team goes above and beyond to assure that everyone is cared for like they family, and indeed, it is because the patients are in fact more than just a patient - they are Seasons Family and cared for as such. I could not have chosen a more caring and compassionate group for my Mom - their recent award from Caring.com is only a hint of the true depth and magnitude of their talents and kindness. Mom is cared for with love and her dignity is assured and advocated for everyday. I only wish that every dementia patient could have this experience and this awesome level of care. I am reminded daily of how lucky we are to have found Mona and her team, and the smile I see on my Mom's face is the validation I need to know we have made the best decision. After all, you only ever want the best for the person who not only loved you best in life but for whom your heart beats. Kudos to Mona and the Seasons Team, for showing us everyday that Those Who Are Forgetting Should Not Be Forgotten. My love and thanks to you all.

I honestly don't know what I would do, if I didn't know my father was at Season's! He had been in other placements and had to be moved, I didn't have the peace that I needed. The owner, Mona has a very high standard and people who work there don't last long if they aren't compassionate and loving, on top of being able to do a great job. The manager Arlene, seems to be able to do 100 things at a time, and yet spends quality time with each client, in a very tender way, and the staff is great! They treat the clients as if they were part of their own family. You won't walk in and smell that "normal smell" like at some places. The food is all home cooked and appropriate for the clients ages, and delicious. The thing I love the most, is that you feel like you are part of the family when your family member is there. I highly recommend Season's, and im so grateful for my dad to be able to live there. I visited close to 30 places, and i can tell you, this is a great place. The patient/staff ratio is highest of any place we toured!! Thank you to everyone who has worked so hard to help my dad!
