A safe place for my brother
My brother [name removed] was suffering. I needed a skilled nursing home for him. I wanted it to be accessible via public transportation. The first two homes I sent him to were nightmares. I was at a loss after the second one failed. I had to get him out of their, but how do I visit him? That's when I started thinking about my own personal stay at Heartland of Mentor, which is now Primedica of Mentor. To my wonderful surprise when I called out there I talked to a familiar voice. A therapist of mine when I was there was promoted to admissions. Immediately right then and there I realised if my brother had been there my worries about his safety and care would have never been an issue. And I arranged transportation with a good friend of mine to visit him on Fridays. When I did I saw a lot of the wonderful peoples who took care of me so many years ago. If your reading this and looking for a place for your loved one please take this review seriously they are GREAT and WONDERFUL people!!!

Kind and caring
Every time I visit I am impressed by how much they care for and care about their residents. They also will stop and answer any questions I have my family member is happy and comfortable he especially loves the food.

Sub-Par Patient Care and Safety
I have no words....only 1 star because I have to click something. I am a hospital administrator and shutter at the patient safety and compliance at this facility. The nursing staff is minimal and they all seem to have "call-light" fatigue. Administrative response is poor and dismissive, and finally, interdisciplinary communication is of serious concern. Patient Care & Safety people! Patient Care & Safety! It should be your priority at all times. Heartland barely gets by and slides in under the radar for patient safety compliance and regulatory standards. If I were CMS, Ohio State Board of Health and Joint Commission - you would be shut down until you made serious Quality Improvements. You have a few wonderful PT, OT and Nursing individuals; however, an overall rating of a 3 out of 10 facility. The 3 is given for those individuals that work there and actually take accountability for their personal medical practice.

They are a wonderful place. The therapy and rehab that they provide are the best. We love it here and things are going the best that they can. Everything has been going excellent and I would recommend them to others.

Heartland is a DISASTER!
Heartland located in Mentor, Ohio is a rehab facility which completely neglects and mistreats patients. My mother was a resident there many years ago and received wonderful care from knowledgeable, compassionate, and caring professionals. Unfortunately, I cannot say that for her second stay as a rehab patient who had back surgery and complications from a stroke. The staff is unprofessional and provides little to no support for many of the residents. They are continuously rude to the patients and their families, often mocking them, and even NOT following specific physician instructions such as properly dispensing medication, suture removal, and practicing safe guidelines to protect patients from injuries such as falls. My mother was [removed] by the staff on the day she was admitted, and no one was held accountable for the contusions she suffered. The facility is not cleaned regularly, especially the patient rooms, private bathrooms, and dining halls. The furniture is dirty and in disrepair, and has not been changed since her first stay as a patient(about 10 years). They are greatly understaffed which explains why patients are often left waiting for extended periods. The food and the dietary services are simply awful, and the social worker rarely spends time with the patients or assists the families for the next step which for many is returning home. I would not recommend this facility for a loved one as they are NOT committed to patient care or the physical and mental health of its' patients and residents. Heartland is a disgrace from what it was originally , and I am glad my loved one is no longer there. Please consider another facility before going to Heartland.
