ABSOLUTELY NOT
We were advised by the hospital my mom would need to here. This place is a dump. Filthy linen. Sub-par menu. Cannot work with special needs. If you have Dementia good luck . Staff with never ending complaints on working with a Dementia patient. Be prepared to spend a lot of time here as you will be the caretaker. I cannot In good conscience recommend this place. It's nasty and rundown with insufficient trained staff. Staff is basically there for a paycheck. Not required caretaking for the job. Poor at best. If you are OK with bare minimum for care this is it.

Great Facility Caring staff!
I toured a lot of facilities and the Business Development Director actually took time with us and answered a of our questions, actually cared about our situation. Transition was fast and easy from hospital. The staff was great, had personality and extremely clean. No Smells in the air! Dynamic therapy girls. Beautiful outdoor areas to relax! love it. Food was delicious and homemade. Our dog came to visit too which is important

Wonderful facility with caring staff
My mother was at Phoenix Mountain it was wonderful experience, the staff is very caring and they are under new ownership. They accommodated all of her needs. Therapy here was the best we have experienced and her knee recovery was great. The staff saw her in the hospital too and answered all of our questions. My mother likes soup and it is homemade every day. Very clean also facility!

Phx Mt: Absolutely Horrible Facility
The absolute worse care facility I have ever been to. I would give it a minus “O”. The people were exceptionally rude in every way & the room was horrible also. Not a chair in sight or a place to put your belongings. No walker in the room either & no place for a patient to even walk & as you know, it Is very necessary/important to get up & walk after a procedure. Only a very long narrow walkway to the bathroom. Two people couldn’t even have passed each other or stand next to you to help you. The small TV hung from the ceiling right in front of your face in bed so it was uncomfortable to enjoy watching. No room to move around for anything! Again, a horrific experience in every way. This place should be closed down!

Horrible skilled nursing
They brag that this is family owned and they are local. Owners are con-artists who live out of town & never visit the facility. Place is run down, pathetic nurses who have no experience or empathy for others. Nursing staff is too busy eating donuts and gossiping about their weekend to care for the patients. They roll their eyes when you ask them for anything! Don’t send your loved ones here. This place is horrible.

Nursing home is not a happy place to be in, this one shows why
I took my mother out of the facility as soon as she told me that person called 911 in the middle of the night do to lack of attention, she was in pain and there was nobody helping her, my mother had issues with some of the services, she had to wait to go to the restroom and by the time they got to her she it was too late :( Place is very noisy, equipment (beds) older. After first night, I asked for my mother to be moved to a different room because her roommate had tv on all day and all night. They moved her with person that was calling 911 in the middle of the night, screaming and yelling at the staff cause she was not getting the help she needed. So much for a quiet, enjoyable environment. When I talk to them about the issues, there was no solutions offered . My mother was too tired to stay there another day. On the side note, I was surprised by [name removed] (not sure who she was, owner? manager?) visit to the hospital before my mother was checked into the nursing facility and bragging how good the place is and how they spent $40,000.00 on new equipment (3 times in 15 min conversation) [name removed], compassion, understanding and caring is FREE, teach your staff to use it.

Poor nursing care.
Mom went here for rehab and started having medical issues. Even after issues were pointed out to them they still never addressed the issues. Mom ended up in hospital after I took her to emergency myself. My mom was total care and expected to shower herself. Issues were brought to the director of nursing and still weren't dealt with. The physical therapy department was very good. Some of the nurses seemed incompetent and actually ask me if I believed everything my mother said to me. I did not return my mom there after her hospital visit and placed her in another rehab center.

A Place for Mom has scored Phoenix Mountain Nursing Ctr 1 out of 10 using our proprietary review score.

We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.
