Steer clear
My mother came out of CareOne at Wall in worse shape than she came in. She spent three weeks in the hospital with sepsis after a foley was backed up and neglected, causing an infection which spread to her kidneys. This resulted in a life-threatening sepsis infection compounded by Covid. The staff was insistent that Mom must have gotten the infection in the emergency room, despite the obvious progression to almost her lungs. This negligence also effected Mom’s therapies, which were nonexistent while we paid out of pocket costs. After a care conference during which we agreed to pursue therapy options covered by insurance only, we were hit with a surprise bill for those therapies - turns out the staff proceeded without seeing what was approved under her plan, and without consulting us on what she was receiving that would not be covered. The lack of communication is astounding, as we consistently had to request updates multiple times before we received responses.

Not a nice facility
The staff, for the most part is wonderful. There are, however, several age who have no care or compassion for the patients and don’t answer the bell. I can’t say enough about the nursing staff. They have been terrific, and I’m a crotchety old man. The place is dirty. There were bugs in the drain in my shower each time I try to take a shower, which is twice every dollar spent on maintenance or cleaning, is a dollar out of the greedy owners pocket, the foods awful I hope you can find a different place

Avoid at all costs
Avoid this place at all costs if you care about your family member. The place is dirty, outdated, and needs lots of repair. The food was disgusting and they never got the order right for my mother. I heard every excuse in the book from they ran out of something to the bananas are green to they forgot the yogurt and the coffee was cold every morning. The care was pathetic. If I heard one more time they were short staffed I was going to throw up. I don't understand why you would take a patient if you're short staffed and can't give them what they're therefore which is therapy. We have been waiting for a callback from two nurses for two weeks to get updates on my mother but now she has been released which they didn't wanna do because Medicare pay for additional two weeks so they said why would you take her out. The staff could care less administration could care less and I believe if you stick your loved one in the middle of the highway they will get better care than at care one.

Excellent Care and Wonderful Staff
My aunt was in CareOne at Wall for over a month. First she was in rehab then her condition worsened to the point that she was on hospice care. I don’t even know where to start to describe the amazing care she got there. We looked at a few facilities & decided to keep her in Wall because of all of the wonderful nurses, aides, rehab professionals, speech pathologist, social workers, etc.. in Wall. Their compassion & expertise are exceptional. The facility is bright & clean & everyone always has a smile on their face with a positive attitude. They also have excellent Covid protocols. Our family has a great deal of experience with long term care facilities & this facility is one of the best we have seen. The facility is very well staffed & thank you’s go to everyone for keeping my aunt comfortable, clean & pain free; & helping us go through such a difficult time. CareOne at Wall is an exceptional facility & I highly recommend it.

The worst
I feel like I made the worst mistake of my life sending my loved one to this place. They were incompetent. Everything that appeared well wasn't well. God only knows what was happening. I will never recommend this place to anyone. Even the physicians are incompetent. I regret not demanding my loved one be sent to the hospital instead trusting and having faith that as soon as I could get her out of there things would be ok. One minute I was told one thing and then the next it was a total different story. She had a wound that had turned worse and noone told me. She had swelling (lymphedema) and they were giving her diuretics which only induced her kidney failure. I had to ask for care for her. Things that were common sense. I spoke to my aunt 1 hour before they called me telling me she went unresponsive And not even 10 min had passed from that call they told me she was gone. Please, Please, Please do not send your loved one here. I regret this place every second of every minute. Im over 1,000 miles away and it angers me that I couldnt be there to see what was happening. My aunt was walking and independent before her hospital stay and when she got to this place she became almost bedbound and incontinent. I told her to refuse the lasix and next thing I know they came and crushed her pills as if she wouldnt know the difference. They would leave her in a chair for hours with a wound. At this point I talk to her every day and tried leaning on faith that it would all be over as soon as I could just get there to get her out of there. If you have a loved on please dont send them here and if you do or have no choice please stay on top of them every single day. Follow your gut if you feel there is a problem and get them out of there.

DONT GO HERE
My grandfather recently passed away at this facility, and it was a horrific experience for him as well as us. The nurses don't care and don't watch their patients. The first night I went to visit he was in a completely soiled diaper with six inches of padding under his butt to soak up excess urine. They just left him like that, for how long, God only knows. Also he was discharged on medication to Care One, that night I noticed he was acting very fidgety so I when I looked for his nurse (by the way it takes 10 minutes to a half hour to find anyone that works there after 8 p.m.) to ask when he received his medication last and when his next dose was I found out not only had they not given him any that day, they didn't have any in the entire facility to give him. They had to call someone at 1 a.m. for medication and only one out of the 2 that he needed arrived. The nursing staff there are very cruel and uncaring. Even in a meeting with the admin my statement that the nurses must be desensitized to the needs of patients was confirmed with the nod of a head from the social worker. Stay far far away from this place.

Stay far far away
The absolute worst experience of my life was to watch my grandfather suffer his last days at this place. The nurses have become so desensitized to death and the elderly they are cruel and uncaring. When they don't know you're coming they leave your loved one in a bed with a soiled diaper on and towels under their butt to soak up excess urine. My grandfather was released to this house of horror from the hospital on two medications that they didn't have in stock and would never have gotten it for him if I didn't push and advocate for him. I'm traumatized by what I witnessed at this place, I wouldn't recommend it to my worst enemy.

House of Horror
This place LITERALLY tried to kill my grandfather. He's on comfort care with pneumonia, ON CHRISTMAS EVE, on his very first arrival date from the hospital. I went to visit him early this evening, I stayed until visiting hours were over and then left. HOWEVER..... my mother and stepfather were on their way. But they didn't know that. I gave my mother the security code to the elevator, and what did my mother and stepfather find??? Nobody. Nobody working the floor, the nurses station abandoned. But when they went to my grandfather's room there was my grandfather.... lying bare on his bed, no clothes, no blankets, no comfort, and with the WINDOW OPEN!!! It is 35 degrees outside!!! They spoke with a cleaning lady, she was the only person they could find and she barely spoke English. They covered him up and closed the window Then they ran home I went back there to confront them then called the police. The police came and investigated, my grandfather pointed and told the police they opened the window. He was crying and squeezing my hand for help. This LITERALLY just happened. On Christmas Eve. I'll be back all night tonight checking on him, and then getting him the hell out of there. If you LOVE your loved ones, DON''T EVER GO TO THIS PLACE. 12/29/2018: They desperately want me to change this review.. okay I will. Now that my grandfather has passed. This place is horrific. He was too weak to move, so I had to pretty much move in and stay by my grandfather's side 24/7 to ensure he was cared for. The first night, after the initial review, the nurses said they had to change him and asked me to step out of the room. I said absolutely not. They looked at each other and then tried using their body to hide the completely soiled diaper and 6" off padding under his butt to soak up excess urine. They were very harsh moving him as he screamed out in pain, even though I was standing right there. Then when I saw he was very fidgety I went to the nurse and asked when he was given his medication last. He was discharged from the hospital on ativan and morphine. They had no medication to give him!!! They never planned on giving him any. They had to call someone at 1 a.m. for medication and I held his hands for an additional 2 hours while he groaned in pain and fidgeted with his blankets until it finally arrived. The next few days were hell. The nurses are cold as ice and cruel to their patients. If you need one at night they are nowhere to be found. I firmly believe they tried to kill him. Why? I have no idea. But thank God my mother and stepfather came late that night and saw what they did. He is too good of a man to die like that, nobody deserves to die like that. Like I said before, if you love your loved one, and want them to be cared for, THIS IS NOT THE PLACE TO SEND THEM.

No care at Care One in Wall
My mom was there for rehabilitation and recently passed away. The care was horrible ,food was delivered and taken away without anyone checking to see if she had eaten. She went to the the hospital dehydrated, malnourished, and with horrible bed sore that needed to be treated. They are understaffed and it seems like everyone is heavily medicated so all they do is sleep to make it easier on staff. I only came across 2 nurses their that seemed to even care. I would never recommend this place to anyone. The place looks beautiful when you walk in I think they spend money on facility and not enough staff to care for the people staying there. It’s very sad to see elderly people begging for help and to be ignored. Absolutely horrible!!

This is the most awful place to put your loved one. Do not go here! My reasoning is the following- Some of the aides were more interested in there own personal lives rather then taking care of the patients, always sitting down the hall way on cell phones when patients are in need of assistance, or advising its time for my meal. Nursing staff sure liked to call the families when they could not handle patient, but instead over medicate the patients so they would not bother them. Also when asked about bruises and a black eye, they don't seem to know how it happened. Admin staff/Social workers, give you the proverbial "will call you back and never do. And the best, once the patient is discharged, beware they may need another hospital admission from an infection. Will never ever use this place again!

Sucks Balls
Checked few sub acute rehab on a Sunday afternoon. It was calm and quiet. Totally different story come go time.Dont be misled by the nice crown moldings and tile floors. Horrific care. Overworked and they let you know it. 90% of them were bothered by any sort of request such as a towel or asking to clean the dirty toilet. I got my wife out in 2 days . No way in Hell was I leaving her in there for the full stay. Had I ignored the warning signs, the first 5 days would have had her sitting there with putting a dent in her mattress sucking up her medical insurance. If you dont listen and do stay, pay attention to your meds that come late. You may mistakenly not get your pain meds And if your loved on doesnt have an appetite, they may not eat that day. Ive seen full trays of go back. Ive sincerely seen food at homeless shelters better quality and served with more care. The only person that earned her pay and did the job of two people was the sweet woman working the front desk. Im not one that is snooty or likes to complain for the sake of it. Im telling our story to help you make an informed decision

They almost killed my grandmother
If I could give a zero I would. Let me tell you how they almost killed my grandmother today. My nana has copd... she is on 2L continuous oxygen and a steriod breathing treatment every 4 hours. Well today she was moved to a different room, after the aid wheeled her in and left her unattended, no one put her back in oxygen. She rang the bell for 2 HOURS before someone came to fix it. When they fixed it, the aid accidentally put her on 4L. This dose could literally kill her. (Also she has not had one breathing treatment even though she’s supposed to have them every 4 hours). Luckily she is still with it cognitively to know something was wrong and told her PT something didn’t seem right. They corrected her oxygen and never even documented any of this in her chart! This place should be ashamed of themselves. This issue is on top of the fact that they haven’t had her medical diet listed properly for over a week no matter how many times it was addressed. I feel bad for the patients that cannot advocate for themselves here. Although social work is very nice and there are some very nice nurses; today their errors could have killed my grandmother!

She was happy. When care was able to take her on her second rehab visit. I think the care is really good. They were very good, any questions I had they were able to answer. I thought the nursing staff was great and they were well educated. Everyone was very friendly. Even the kitchen staff would greet you. It’s a very lovely community.
