My mother is having a hard time adjusting to this community, but I do feel she is in good hands, This is a good community with kind people . The staff is kind, and trying their best to help her adjust. They have nice activities, and good meals available for the residents. They do a good job keeping the community well maintained. It is a nice community, and it would get my recommendation.

My father is in dementia care program at Legacy. I have never had a bad interaction in over 1 year. The staff is caring and attentive., managers are walking around. The facility is clean. Decor is cozy. I cant care for my Dad, but there is nobody else I would like to. They also mix dementia people with regular during socials. We all feel great our Dad is here!!!

The staff has been very helpful and nice to my father. He really seems to enjoy the meals that are provided. If he needs an accommodation for certain foods they work with him, so he gets the meals that he likes. It's a safe community with various activities offered. I really like that he has been engaging in activities and making new friends.

Assisted Living

Assisted living communities provide residential communities and health care for your loved one who need help with everyday activities such as mobility or cooking meals. Assisted living communities frequently offer help with appointments and transit, assistance with the activities of daily life, and supervision of care. In some situations, communities may provide hospice, nursing care, or “aging in place”. Wisconsin has specific standards associated with this type of community. The Wisconsin Department of Health Services, Bureau of Assisted Living, Division of Quality Assurance, regulates community-based residential communities (CBRFs) governs this community type. Staff members obtain training in a variety of subjects. These areas include needs assessment of prospective residents, job responsibilities, standard precautions, and reporting abuse. This type of community is required to offer a number of services. These include activity programming for persons with dementia, health monitoring, leisure time activities, and including personal care. This community type has a number of provisions pertaining to medication assistance. These provisions include If the medication administration is supervised by a licensed healthcare professionall, the community must ensure that these individuals coordinate the medication administration process and system, Residents may self-administer medications if they are capable, and when medication administration is not supervised by an RN, practitioner, or pharmacist, the CBRF must arrange for a pharmacist to package and label a resident’s prescription medications in unit dose. Public assistance is available through The Elderly and Physically Disabled 1915(c) Waiver program, the Community Options 1915(c) Waiver program, and the Medicaid managed care 1915(b)(c) Family Care Waiver program for the aged and physically disabled. There must be one toilet for every 1 residents and one bath or shower for every 1 residents. This community has a number of provisions concerning food and diet which include these meals must meet residents special dietary needs, these meals must meet the recommended dietary allowances based on current dietary guidelines for Americans, communities must provide at least three nutritious meals a day and a nutritious snack, and therapeutic diets must be provided if ordered by a physician. Before a resident is accepted, they must go through a service plan screening to guarantee their needs are going to be met. This screening includes a number of areas of interest such as initial comprehensive mental and physical assessment, reassessments must be conducted at least annually, and when residents have a change in needs, abilities, or condition, community must develop a temporary service plan to meet immediate needs, and the plan must specify which program services will be provided to meet the resident’s needs as identified by the assessment and the frequency with which each service will be provided.
