The environment and the people were great. I couldn't have found a better place for my husband. 5 for sure. It was one of the cleanest facilities I've seen. Yes I would say it's a clean, safe community with a friendly and caring staff. Also [name removed] who helped me get [name removed] admitted was wonderful to work with and she helped me out so much. I would definitely recommend Liberty Place to family members looking for a place for their loved ones with memory issues.

We chose OSL Kettering for Mom (her choice, actually, but we all agreed) because they’re different than the typical AL setting. The building is laid out like a house - nice and small with only 12 bedrooms. Residents share 2 common living rooms, family-style meals at a large dining room table, and an open kitchen for all to see the food being prepared. Bedrooms are comparable in size to other AL environments. The staff are all friendly, caring, and are starting to feel like family to us all. We needed a place that accepts Medicaid after 2 yrs private pay and would use a hoyer for transfers. All the beds are Medicaid approved so Mom doesn’t have to move to a “Medicaid room” when her funds run out. We also love the straightforward flat pricing and that her expenses won’t increase as she needs more care. The real winning factor though is that she will not have to move to a SNF if she starts needing more skilled care. They are truly set up to be her forever home! We love OSL!

I would highly recommend Optimized Senior Living for your senior needs. It is a family owned assisted living that caters to the needs of their residents. It's non-institutionalized and feels like a home. The food is cooked in the kitchens and the care-giver ratio is low. Amazing staff that truly cares for the well being of not only the residents but their families.

We would like to recommend Optimized Senior Living for your loved ones. The caregivers are loving, respectful Christian professionals. The care they provide is extraordinary.

It is a great place, It is small, there are 16-18 residents in each building. The food is good. The room sizes are good. The bathrooms are too small for someone in a wheelchair. They have patios for outdoor space.

This place is a gem. Truly a life saver for care. I had looked at many places for my Mom. Not only is the house beautiful but the care she has received has surpassed any other very expensive place she had been cared for in. Her room is very large and she has her own furniture there. The staff will cater to her dietary needs and she has stayed very healthy. We love the fact it is more like a home, not an institution such as a standard nursing home. They are great at helping to solve any concerns she has. I would urge anyone seeking long term care for a parent to check this arrangement out.

The staff at Angel Care couldn't have been any sweeter, patient or more helpful to my grandmother during her year living there. They did everything they could to provide her with comfort and support. I am very grateful for all that they did to not only take care of her needs but to provide constant communication and support to my mom who lived near by. My grandmother spent her last days in Angel care. The love that they showed her during her last days still warms my heart. It was very obvious that the residents are more than just patients to the staff. I will always be grateful that we were able to find a loving facility that truly felt like a private home. Without a doubt I would recommend them.

I am happy with the staff at this service. We got a great caretaker that is so nice and helpful to all the needs for in home care. The menu is appealing. and she likes the healthy choice. The activites are very helpful for her needs. We would highly recommend this community for the care and the good value for a loved one.

This is a much better fit for my loved one. They have great caregiver to resident ratios. There have been changes with Optimized, some good, some bad, but still overall a great concept. They have facilities all around the area, so I'd definitely look into one.

The facility seems to have an issue keeping staff but that's the same at any community now. The room sizes are very good, the food looked and smelled good and they offered a good variety. I would recommend this facility to others.

Everything is working out pretty well for my mother here. She is still getting adjusted, but I know she is in good hands. This is a small care home with a great design and concept. The house is a ranch style with the kitchen, dining room and gathering space in the center. There is a small bedroom behind the kitchen and 3 larger bedrooms on the left and on the right off the main gathering area. Each bedroom has its own bath. (The house will hold 6-8 patients.) The responsive and friendly staff care about the men and ladies who live here. They make their beds, give them showers, help them dress, do their laundry, and make their meals. If time allows, some will do activities like puzzles or games. If you're looking for a caring atmosphere, extra attention for a Dementia or Alzheimer's loved one~this is the place!

Beautiful facility and great concept for Senior Living. I think more staff would be my recommendation. Also a cook to prepare meals instead of the aids cooking and taking care of the residents.

I toured here when looking for a place for my Mom. It’s a very small facility; I think there may have only been 17 total bedrooms within the whole community. We weren’t comfortable with how our tour went. The representative that was supposed to meet us wasn’t able to, so a family member of a resident that was already living there took us around. They didn’t know a lot about the facility or the type of care that my Mom would need. However, the facility itself was very clean. They only have one room units per resident so it’s laid out more like a studio apartment than a one bedroom.

Beautiful and quaint. Liked private rooms. Concerned that they are not licensed by state. Licensed through MRDD. Will keep this in mind while continuing my search.

I toured this care home for my husband, and his needs. My tour was a nice one, everything went well. From what I saw the home appeared to be well maintained. The staff I met was kind, and friendly. The experience went well, but I didn't feel this was the care home for my husband, and his needs.

Before my mother was taken in, Optimized in Lebanon wanted $2,500 to hold a bed and the first two-months rent($6,000 per month) After paying Optimized $14,500 my mother was there for two-days in a vegetable state. My mother ate NO food, had NOTHING to drink because she was completely unconscious because of the morphine being given to her by hospice. Mother passed after two-days & all the family received back from her two-day stay at Optimized in Lebanon, Ohio was a check for $6,000. My family was out $8,500 for two-dreadful days while at Optimized in Lebanon, Ohio. On my first-day visitation I was also confronted by a dog INSIDE the facility & though the dog didn't bite me I can willfully say it scared me. The next-day I visited my mother again and upon entering the facility there were now two dogs inside the facility only to see one dog trying to climb atop the other trying to mate. Simply a very poor place for a loved one

Place is ascam, go anywhere else. They smoke cigarettes around the residents. The place is dirty, they never clean the rooms. My grandma slept in mattress soaked in pee for over a week. They let a resident fry and get sun poisoning. They are occupied with Personal phone calls and smoke breaks that my grandma wondered out the front door on the a busy road. They made her eat dinner by herself. The food was cold and cheap for what they make you pay to live there. Talking frozen pizza and stuff that look canned cat food with potato chips. They feed them the diet of a 6 year old. They never have the lights on during the day. There’s is no activities unless there is a tour coming they will put on a show. Basically the residents sit in chairs while the staff watches tv for themselves. The hair salon is a joke it’s basically a trash bag draped over the resident . Putting your love one in that home will take away their dignity. Place is joke needs to be shut down. It’s a waiting room for death.

I didn't like the rooms at Optimized Senior Living. And ultimately I just got an off feeling that something wasn't right, but I couldn't put my finger on it. . .
