Review for Grand Brook Memory Care of Carrollton
We picked this community for a couple of reasons. The main reason is that they offered more services like being willing to feed our family member if they could not feed themselves, or being willing to deal with incontinence and adult underwear. Many of the other facilities told us we would have to move our family member if those situations happened. We also like the fact that this was a memory care center and not a combo facility with assisted-living and memory care.

Great Experience with Grand Brook
I have found the Grand Brook staff to be quick to communicate and very helpful. The activities director, [name removed], is the best AD I've encountered at any memory care community. My mom has fallen less since moving to this community, and seems overall happier.

My loved one is currently a resident at Grand Brook Memory Care of Carrollton and everything has been very good for him at this facility. The friendliness is great. The cleanliness, staff, meals and activities are good. The value for cost is great, compared to other Memory Care facilities in the area. Compassionate Care with Great Staff
My mom has settled in so quickly and seems very comfortable. I appreciate how the staff has treated her as a unique individual and allowed her to be herself (they don't force her to get up early or eat in the dining room). The staff is kind and respectful. They make it very easy to visit.

Above and beyond staff
I am very happy with the experience at Autumn Leaves. It is the perfect fit for us. The moment we walked in I just felt that this was the place, it was so homey and I immediately had peace of mind. The staff from management to the cleaning staff, they are all amazing. They all know my name when I go to visit and they truly care about the residents. They are very patient with her and every time I am there they are engaging with the residents. My mom has made a lot of friends with the staff. She has more of her memory than the other residents, so [name removed] the activities director has her help with different things. She is very picky but loves the food, we were not expecting that at all. They are associated with the little lantern clinic next door, so the doctors and nurses come over so we do not have to drive her to appointments. They have been so helpful and accommodating and made our move over here go smoothly.
I would highly recommend them to other families, it has been the perfect fit for her.

We love this facility!
We had a very good first impression, Autumn Leaves is a smaller community which we really liked, and it is all on one level. The staff was very friendly and welcoming. The rooms were fairly big. Our loved one has yet to complain about the food, and they are willing to accommodating any likes and dislikes when it comes to the meals they serve.

JUST DONT DO IT!
My mother was living at autumn leaves until April 2020. Yes, [name removed] was the Director at that time. During the lockdown I was told she had a hip fracture and needed a hospital bed and pain meds. 24 hours later she was pronounced dead - and ALL OF HER JEWELRY HAD BEEN STOLEN! Neither [name removed] NOR any of the "family" owners can say what happened - Even though only employees could enter! Three months later - NOTHING! What happened to the investigation you promised [name removed]?

Great Facility
My mother-in-law has Alzheimers and is currently living at Autumn Leaves of Carrollton. She LOVES it there. They take her on outings and she participates in all kinds of activities. Friday afternoons they do Karaoke and the families are encouraged to come and participate with the residents. Families are welcome to participate in any of the activities that they do with the residents. The staff is very friendly and my mother-in-law consistently comments how "nice they are here." The only issue we've had is their laundry service. They keep misplacing her sheets and comforter, but they are always willing to look for it when it turns up missing.

Peace of mind
Out of all the places I visited, I liked this one best of all for my husband with dementia.
We are truly like family now, and although every place has their issues & concerns, Autumn leaves staff will work with & fix any issues we may have.
Always a great atmosphere & fun activities for the residents as well as family events.
The food is delicious as well as nutritious & they make sure they get what each resident needs, I’m really thankful I found this place so I can relax & know my husband is well cared for.

WE LOVE AL of C!
My mom has lived at AL for 2 years now and we feel so relieved to know she is in a safe environment. She was [removed] in a previous memory care facility that was TOP rated, well known and very beautiful. AL of Carrollton is quite a bit further from our house, but it's so worth the drive every day to know she is safe, well cared for, and in a lovely home setting with a gentle, loving staff, wonderful activities, terrific food and a welcoming environment. We love the caregivers, administration, activity directors, chefs, the front office heroes, and the maintenance man and the housekeeper (who have both been known to grab a spoon and feed residents who are struggling to feed themselves). AL of C is truly a family taking care of family. I can't thank them or recommend them highly enough!

This is a great community for the care for my mom.The staff are attentive and caring to her needs. The menu is good and she is happy with the variety of the meals. They have a lot of great activities for her to participate with the other residents. We would The community is clean with a nice environment for her to enjoy.

Autumn leaves provides a loving environment for their residents and support for the family
Words cannot express my gratitude to the wonderful staff at Autumn Leaves Carrollton. I had been struggling to take good care of my mother. From the moment she arrived, it was evident that she would receive excellent and loving care. They treated her like one of the family. When she transitioned to hospice, the staff continued to come to visit her and to help the family through the difficult times.

Old style layout!
The Autumn Leaves of Carrollton was a nice bright place, it had an old style layout. The place was clean and everyone was very nice. The thing I did not like about this place is that there were long hallways and all had to carpet. I was looking for something more homelike and rounded.
