Our memory care community, also known as Clare Bridge, is rooted in “a person-centered approach”. That means we help our residents find a sense of belonging and purpose while also helping them preserve who they are. We also have our Clare Bridge Crossings program which is designed for individuals in the early stages of Alzheimer’s or dementia. Along with our programming, you’ll have peace of mind knowing that trained staff is on-site 24 hours a day, seven days a week.

Here at our community, we help residents stay engaged with a gentle daily structure of planned activities to help them maintain their abilities and encourage the use of their current skills. Here are a few examples of your family member can enjoy here at our community:

The Clare Bridge Daily Path is an evidence-based approach that offers a gentle daily structure of six planned programs per day, our routine that helps our residents maintain their abilities, encourage the use of their current skills, and offer a sense of security and accomplishment.

People living with dementia are at a greater risk for poor nutrition and weight loss. That’s why we use mealtime as an opportunity to offer healthy menu choices and create a sense of belonging. Our residents enjoy dementia-friendly menus with familiar and easy-to-manage selections, small dining rooms with consistent pre-selected seating and personal dining assistance.

Our community environments feature areas of the community that are set up for safe engagement in gardening, kitchen work, handyman help, and other household and normal daily tasks. Life skills activities provide a sense of purpose, meaning and belonging as residents complete tasks related to everyday life.

Residents can enjoy spending sunny days outside in our large courtyard or meeting up with friends in the game room to work on a puzzle. Or they can join a cooking demonstration in the community kitchen. And residents love our convenient on-site
Whether it’s our Crossings program for the early stages of dementia, our Clare Bridge program for mid to late stages, or our Solace program for the advanced stages of dementia, we have purposeful programs that offer the care you and your family need. Our exclusive Personalized Service System allows us to provide individually customized plans for your family member.  Your customized plan means you only pay for the services you need and want. And we’re flexible, so when your needs decrease or increase, your plan and its cost do, too.

    They provide of wide variety of supports. The caregivers are very friendly and they seem to genuinely care about my mom. We chose this community because offered the three levels of programming which was more a cognitive appropriate setting for her and they offered what she needed, The location is close to family. I would recommend this community to other families.
