Had the worst experience
Not enough staff. Had to voice concerns on multiple occasions. Had to ask for help every visit and I was there everyday the time My Mother was there. The Head Nurse was on a Power trip. Did not like the way She or I was treated by staff. Would Not recommend this place to anyone. First time to experience this kind of Facility and had the worst experience.

Gross negligence caused my mother her life.
Multiple issues here. Mom left in bed over night crying in pain (because a nurse refused to help her up at all), she went two days without her medications, and after I (her caregiver of 8 years) told them something was wrong/ her numbers had to be off/I thought she was septic… the staff and Dr. [name removed] ignored me leading to her losing her life unnecessarily 5 days later due to them thinking “blood work I requested was not necessary” and if I was “so worried I should call an ambulance”…. Well I did, but they would not let me talk to my mother an entire night before, during, or after the ambulance I called got there- so they left it up to a sick and incoherent women out of dead sleep with no context decide if she should go on an ambulance or not. Needles to say she had no clue what was going on so she didn’t go and it allowed a deadly infection they mis diagnosed to grow 30 more hours than necessary and gave her meds that made it worse… Don’t do this to your family please!

Nitemare!!!
My mother was transferred there for short term rehab (20 days) from the hospital after a lengthly illness to gain her strength back. after 4 days we began to frantically search for a new rehab to get her out of there out of pure fear for her safety, twice we found her in bed soaked in urine once with feces she is now recovering from a "pressure wound on her elbow that I was lied to about , I was told is was just protection so she didnt hurt it during her rehab. I was shocked when my mother called me for help since no one would come help her to the bathroom even when i arrived (30 Minutes later)her call light was still on and no staff to be found, and NO.. nobody ever came into her room but the call light was turned off eventually. I was told by a staff member that they were told she had no family or visitors so noone seemed to notice us either! that is just a few of our issues with this place. Protect you loved ones DO NOT leave them there!

Decent care but, theft is an issue that is ignored
The physical therapy team is top notch. The nurses aides were great and very personable. There were delays (especially at night and possibly due to staff shortages) that led to falls since my dad was trying to make it to the bathroom in time. When he was admitted, he had five complete outfits. They all disappeared. I bought an additional eight outfits. They also disappeared. When he was discharged, those outfits and his personal blanket and heating pad were missing. Management was aware of the issue. Nothing was found or replaced. They did not offer reimbursement either. Another issue was with one of the receptionists. The website clearly stated on the front page that appointments were no longer required for visitors. My daughters went to visit their grandpa and were very rudely griped at for not having an appointment. They merely went by the information provided by the website.

Careful review of this facility's management & staffing patterns is needed!
After a prolonged hospitalization which left her in a severe weakened state, we transferred our elderly family member to the Plaza in hopes that the skilled nursing services (PT, OT, Speech) would be of benefit. We found the immediate floor staff and therapists to be courteous and friendly. In my opinion that can never make up for under staffing and dishonesty! The facility demonstrates little regard for patient safety by depending on a call system that is only a flashing light above the door and can only be seen by someone actually in the hallway. The hallway desk was routinely unmanned, especially in the evenings! (How about a cheap wireless doorbell installed at the hall desk or main nursing desk) We also found severe deficits in the actual management of this facility to the point of incompetence. The floor staff on the hall which consisted of 1 direct patient aide and 1 LVN to give meds who said she was in charge and denied knowing who her supervisor was, refusing to give me a name of someone I could contact about the situation. She also lied to me about the 1 aide's availability to help my family member after repeated requests for assistance to return my relative to bed since she was complaining of extreme tiredness and pain. The situation was one in which I could not physically move her myself or I would have! The LVN offered no help other than saying the 1 aide was with another patient which turned out to be a lie. The aide was off the floor for her own personal reasons. That left my relative without assistance for over 40 minutes. There were several other deficiencies that discouraged confidence such as apparent poor communications between various staff members, lack of attendance to basic room supplies and a general sense that good employees were being taken advantage of by poor management practices.
