Your loved one will have a care team dedicate to all of their care needs. This team is built up of a Care Planner, a Care Coordinator, and however many Caregivers are needed to give quality service. We focus on keeping the teams as small as possible while allowing for sick days and vacations. This allows you and your family to receive consistent, quality care from someone you know and trust. Your Care Planner is a single point of contact that will continually evaluate your needs and be there for any questions you might have.
Navigation and only as much care as needed
Right at Home works with you to ensure that you have looked into government support, insurance, Veteran Affairs etc. Our Care Plans are always changing as we constantly review your needs to ensure you are receiving the service you need and only what they need.
This unique program is a monthly subscription based program that allows your loved one 24/7 telephonic access to their own personal nurse using their own personal medical information. It is a very cost effective virtual support program with a low monthly fee, and covers such things as: scheduled medication reminders, hydration calls, booking medical appointments, appointment reminders and consolidated notes. Mom or dad can call in the middle of the night and triage any concern they have. A nurse will use their records to help guide the client to the best solution, reducing 911 calls and hospital re-admittance. If a hospital visit is required, the RightConnect365 team will send all relevant files to the hospital. Families of the client have access to the portal so they can be comfortable knowing what's going on with appointments, even if they are far away.
