5 stars for all. I was so impressed with Autumn Trace in Columbia City and all of the staff that I stopped looking any further, I knew that this was the place. The location of the facility, the newness, the meals, caring staff and management are what sticks out most about this facility that has made it such a good experience for my loved one. I would absolutely recommend Autumn Trace to others. It was a perfect fit for my 102 year old step dad.

I highly recommend Autumn Trace Assisted Living in Columbia City, Indiana. They provide a safe, clean, and healthy environment for the people who live there. I am impressed by the way the staff all work together to provide the services that the residents need. They have a nurse that will set up medications and staff give medication reminders and supervise medication taking. They allow small pets. They have an inexpensive room that family can rent when visiting. The food is good and you can select what you would like to eat from a daily menu. My mother is comfortable, talks about how friendly everyone is, and she enjoys living there.
