My mother has had a wonderful experience at New Perspective. After a fall breaking her leg at home she needed a place with rehab and could accommodate her living needs. I am thankful to say that after several months she is ready to go home and travel again. She can't say enough about the staff and residents making many new friends. I have had my aunt at Brookdale Century Ave for the last 4 years and my mother has been there for the last 3 years. We have had a great experience throughout the entire time. The staff has always shown a great deal of care for all of the residents. Up until the Pandemic I have spent a lot of time visiting this facility and have never seen anything that caused me to be concerned. The meals are of good quality and served promptly while hot. My mother has even been asked if there is a meal she would like to see served. The whole environment seems like a big family. Residents are definitely treated with respect and care.

Monona, Wisconsin

Assisted Living

Assisted living communities offer organized living communities and health care to your loved one who may need a helping hand with everyday activities such as self-administering medication or dressing. The services offered often include aid with ADLs. Inhabitants may be kept under monitoring and assessment. In some situations, communities may provide dementia care, hospice, or “aging in place”. Wisconsin maintains a number of provisions related to this kind of community. The Wisconsin Department of Health Services, Bureau of Assisted Living, Division of Quality Assurance, regulates community-based residential communities (CBRFs) regulates this type of community. This community has a number of provisions concerning food and diet which include communities must provide at least three nutritious meals a day and a nutritious snack, these meals must meet residents special dietary needs, therapeutic diets must be provided if ordered by a physician, and these meals must meet the recommended dietary allowances based on current dietary guidelines for Americans. There must be one toilet for every 1 residents and one bath or shower for every 1 residents. Staff are required to receive education in a variety of areas. These include: provision of personal care, standard precautions, first-aid, and resident rights. Public assistance is available through The Elderly and Physically Disabled 1915(c) Waiver program, the Community Options 1915(c) Waiver program, and the Medicaid managed care 1915(b)(c) Family Care Waiver program for the aged and physically disabled. This type of community is required to offer a number of services. These include activity programming for persons with dementia, including personal care, information and referral, and health monitoring. This community type has a number of provisions pertaining to medication assistance. These provisions include when medication administration is not supervised by an RN, practitioner, or pharmacist, the CBRF must arrange for a pharmacist to package and label a resident’s prescription medications in unit dose, Residents may self-administer medications if they are capable, and If the medication administration is supervised by a licensed healthcare professionall, the community must ensure that these individuals coordinate the medication administration process and system. Before a resident is accepted, they must go through a service plan screening to ensure their needs are going to be met. This screening includes a number of areas of interest such as initial comprehensive mental and physical assessment, the plan must specify which program services will be provided to meet the resident’s needs as identified by the assessment and the frequency with which each service will be provided, community must develop a temporary service plan to meet immediate needs, and reassessments must be conducted at least annually, and when residents have a change in needs, abilities, or condition.
