Does Mom have to withhold taxes on a caregiver's pay?

The caregivers are part of a registry, not an agency. We hire them directly.

If your mother's check for services is made out to the registry, they are likley responsible for withholding taxes. You should question whether this is being done. If your mother's check is made out to the caregiver, the caregiver would be considered an employee and you should withhold taxes and meet other Federal and state regulations. I would suggest that you seek a CPA in your state to address Fed and state specific issues.

If the caregiver files as self employed do I still need to withhold taxes?
