Professional Referral Partner Program

A Place for Mom (APFM) was founded more than 13 years ago to help families navigate the maze of senior housing options. Since then APFM has grown to become the largest senior housing referral service in the United States and the premiere provider of cost-effective services for our partner communities.

Not only are we a free resource for families, we are also a resource for attorneys, real estate professionals, physicians, discharge nurses, social workers, care management teams and other professionals who work with seniors.

We'd like to partner with you, as one of those professionals, to help as many families as possible to make an informed decision for themselves or their loved one.

Our Community Relations Advisors act as the liaison between your team or family and our Senior Living Advisors, who are the experts on the senior living landscape. When you partner with us, we can help you in the following ways:

  Our Senior Living Advisors can help your patients make easier transitions, something that is especially important for patients leaving inpatient care settings or skilled nursing facilities.

  Our Senior Living Advisors professional team helps families on a daily basis and we want to provide the best local resources possible.
