Located minutes from historic downtown Avon, our brand new community is nestled on beautiful farmland, boasting easy proximity to great shopping, houses of worship, and all of the wonderful cultural and recreational attractions in the area. We put our rates on our website to increase transparency with our customers and their families. We know how important trust is in making this decision, and want to start establishing that from the very beginning. When you consider all of your options, compare apples to apples, and know that many other providers have a lower starting rate and then add many additional costs for things like medication assistance, care, etc. We don’t do that. Our rates are inclusive with no hidden fees. Our dining room will offer anytime dining from 7am-7pm, a Bistro for lighter fare will be open from 10am-1pm. There are numerous common areas including a full-service salon, a media room, library, sunroom and country kitchen. We are excited to be collaborating with The Center for Brain/Mind Medicine and The Center for Alzheimer’s Research and Treatment at Brigham and Women’s Hospital and Harvard Medical School, enabling us to provide the best in purposeful programming and lifestyle interventions to promote healthy cognitive and brain aging throughout our community. It also empowers us to have our finger on the pulse of advancements in dementia treatment, new diagnostic techniques, and innovative non-pharmacological interventions and programming initiatives. As well, we will have access to their clinicians and their studies, provide the community-at-large and family members with educational opportunities, and conduct specialized employee training. Our residents experience the peace of mind knowing that our Residential Care Associates are just a step away to provide support anytime during the day or night. Also, our Resident Care Director ensures a personalized care plan is created and monitored for each resident with the goal of keeping them strong and healthy for years to come.

The Residence at Brookside
"Dad moved into The Village and the transition has gone better
than I could have imagined. He is really thriving. He is more
stimulated and carries on more conversations with me. He willingly
participates in so many activities. I feel confident that he is
eating well, being mentally engaged and enjoying the company of his
contemporaries. I feel a huge sense of relief! Thank you!"

Thank you so much for your assistance with finding a suitable
intermediate care facility for my dad. I got more useful
information from you in about 25 minutes than I did in
hours of searching the Internet and
following up leads with friends and acquaintances over a period of
several weeks while under lots of stress. I wish I found you
