Person-centered care was developed by professional care providers who have worked for many years to improve the lives and outcomes for individuals with dementia. We agree that the best level of care is tailored to each individual. This tailored environment and professional support helps your loved one continue their life with a sense of continuity and comfort.

Sense of Normalcy
A secure patio to stroll, sit, relax, and enjoy the outdoors.
Restaurant style dining
Pub and Café to gather and enjoy visitors or simply relax.
Meaningful activities
Activities like storytelling, exercise classes, tea-time, and pet therapy foster continued learning and community.
Family members are encouraged to participate and help create wonderful memories.
A measure of autonomy
The freedom to simply walk around as he/she likes, which is so common with dementia.
The resident’s care and support are dictated by their life. We meet them where they are. They are not forced to conform to a specific regimen.

Our Tour Experience
They were very friendly to us on the phone, but when we showed up for the tour they weren’t very friendly. They acted like it was a big bother to do it. We tried to remain objective. But in the end, they just didn't’ have what my mother needed (the option for her to be in independent/assisted living and still have accessibility to a significant other in memory care).

DO NOT PUT YOUR LOVED ONE HERE
DO NOT PUT YOUR LOVED ONE HERE! First of all, do not let the owners know if you have LTHC insurance or any other insurance to help pay for the cost of the room and board and care at this facility. If you love your family that has dementia, DO NOT PUT THEM HERE. There are other places that really take care of their dementia residents. Look on the State Licensing Board website and review the complaints against the facility that you are considering. I wish someone would have alerted me before I moved my beloved husband into this horrible, mismanaged, neglectful facility. My husband has dementia, but even he realized in his state that this facility did not have the resources adequately available. His medication was (I would say … ) mismanaged…the med techs gave him overdosed psychoactive medication that put him in a stupor and drooling on his clothing. Even after his physician put in a physician’s orders to lower the doses, that was ignored by the facility Med Tech’s and he was continually drug overdosed until I became aware of what was happening. His basic care was neglected. He lost 30 lbs, wasn’t eating (I believe he wasn’t eating because he was drugged and didn’t understand if he was hungry)…we asked that there be a one-on-one assistances with eating and that we would pay for it, but none was given. Once he was put on those strong psychoactive meds, he became incontinent, eliminating in inappropriate places, and the facility caregivers blamed him. Why would you blame a person who has dementia?? No toiletry assistance was offered him….and they blamed him for not wanting help….he has dementia! The care givers should have compassion and needed to be trained how to provide assistance and dignity to a dementia patient. I would go to visit him, and during the 7 hours I was with him, not one care giver came in to ask him if he needed to use the bathroom, or was thirsty or to come to the dining area for lunch or dinner even after I asked the Care Coordinator to remind the caregivers to assist him even when I was visiting. They said he didn’t want to go to the dining area, but when I asked him or took him to the dining area, no problem. The cook tried her best, probably based on her meal plan. The food was awful, uneatable…so he didn’t eat. I would bring a sandwich from the local bread shop that he would devour. He wasn’t showered until I came to visit and then I would shower and wash his hair. They said he refused a shower when asked……the caregivers should be trained to get around that….these are dementia human beings and should be treated with humane dignity. I have seen the other residents there looking like they were in a daze (stupor) feeling so sorry for them. I could go on…but Please, Please, if you love your family that has dementia, DO NOT PUT THEM HERE. There are other places that really take care of their dementia residents. Look on the State Licensing Board website and review the complaints against the facility that you are considering. I wish someone would have alerted me before I moved my beloved husband into this horrible, mismanaged, neglectful facility. I didn’t even talk about the neglect he experienced. That would be another 3 pages. Go somewhere else..please if you care about your person.

The worst of Chico
The owner [ARemoved], is a horrible person who only cares for herself and not the well-being of the patients. The facilities are worn down and not taken care of. The staff are unmotivated due to the careless management. If you care for your parent, due not allow them to reside in such a horrible place.

As a board member and officer of FDM Community Services, Inc., I would like to take a moment to respond to the comment posted by Chad. First, I would like to be clear about the fact that I have never previously spoken with Chad, through any medium, and that, to my knowledge, no one of that name has toured any of our locations. Our organization takes offense to the description he has provided. I wonder how someone whom I have never directly met, nor who, to my knowledge, has spent any time in our facilities, feels it is appropriate to make such negative comments, and attack a member of our team personally in such a manner. Our company has served our elders for over 20 years, and has always attempted to do what was in the best interest of our residents and our community. I believe our facilities are in an excellent state of repair, and we make a point of taking care of identified maintenance issues as they arise. Further, in my role as a board member and officer, I try to remain involved in the life of our communities, and stay aware of what happens within them, so the accusation that we exercise careless management is yet another unfounded and reckless statement to make. Likewise, in general, we have long tenured staff across our programs, and always do our best to provide a fair, comfortable, and supportive work environment. In closing, I would like to say that Chad's comments are, at best, misguided, inaccurate, and unfounded and, at worst, are slanderous and defamatory.

It smelled like urine and the workers were on their phones while one lady was asking for help and another gentleman was walking around with wet pants. It was very sad to see the lack of care. There were a couple workers that were paying attention but the majority seemed like young adults who had no empathy for anyone and just kept playing around on thier phones. Not a place I would want one of my loved ones to be at. The owners were very nice and seemed to know what to say to appear to have the best interest in mind but seeing how the staff was acting and how unclean the place looked and smelled they don't seem to actually care as much as they say they do.

Active facility with can-do attitude.
This is a wonderful active facility with excellent activities. They are dedicated to making the facility fit the individual rather than otherwise. If there is a problem they work to address it. They do not give up on a resident and simply dismiss them as some other facilities may. There are great activities and the staff are very welcoming of family and friends.

A Place for Mom has scored Roseleaf Gardens 8 out of 10 using our proprietary review score.

We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.

Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
