Pacifica Medical is a CHAP Accredited, Medicare Certified, Licensed Home Health Agency offering the highest level of care outside a hospital or nursing home. Home Care Services are available for private pay, Medicare and privately insured patients.
We provide family focused patient care, including patient and family caregiver teaching, coordination of equipment, supplies, and available community services. Pacifica Medical staff will accommodate weekend and holiday admissions, and provide 24/7 service and nursing oversight.
Pacifica Medical offers many Home Care Services for common chronic conditions and sudden-onset events, and Specialty Programs to prevent re-hospitalization.
Pacifica Medical also provides help for patients needing assistance in performing daily tasks. We offer hourly, daily, or live-in services, matching caregivers with each patient’s needs.
