RN/LVN (Skilled Nursing) Provides nursing care treatment and instruction to the patient and family, coordinating with all others involved in the patient care.

CNA/CHA (Certified Nursing Aide) (Certified Home Health Aide) Assist the patient with activities of daily living PT (Physical Therapist) Helps patient regain functional mobility, increase strength, and improve range of motion. Therapists provide special treatment as prescribed by the physician.

OT (Occupational Therapist) Increase the patients independence and ability to perform daily functions by teaching adaptive methods and use of special equipment.
