Autumn Leaves offers full range Memory Care from a specifically trained and compassionate staff.  Each resident has their own care plan tailored to meet his or her individual needs. We are passionate about caring not only for our residents, but also for their families and friends as well. With stimulating activities, fine dining and a building built specifically to enhance the lives of our residents; Autumn Leaves provides families with peace of mind.

The Autumn Leaves staff includes a full time RN, LPNs, and caregivers who have completed 40 hours of dementia-specific training. Our full time nurses communicate with the pharmacy on reordering medications, monitoring vital signs, communicating with the family physician and his/her staff, determining the daily care plan for your loved one and communicating that care plan with family members. Our caregivers will assist (if necessary) with bathing, dressing, personal hygiene, mobility, meals, and all other activities of daily living. Our philosophy is to help maintain and improve the quality of our residents lives!
