Autumn Leaf is a three bed residential community located in Rex, Georgia, we consider ourselves as an appealing alternative to the institutional atmosphere of larger assisted living communities. We are conveniently located near churches, recreational parks, hospitals, community centers and other entertainment venues that specialize in services for seniors, the disabled and for individuals in need of respite care.

At Autumn Leaf, our caring, loving and responsive staff is focus and steadfast on ensuring the highest level of care. We consider each resident as a member of the Autumn Leaf family, and provide the same level of quality care and companionship that we would expect for our loved ones; it is our number one priority. All staff are screened, and qualified to address health and well-being issues, including trained in CPR/First Aid. We are a State Licensed community that provides 24 hour watchful oversight and RN on call services 24/7. In additional to personal care, health monitoring and medication management, we can also arrange for physical, speech, and occupational therapy on the premises.

Autumn Leaf offers furnished private and semi private rooms; residents are encouraged to bring their favorite photos, mementos, and keepsakes. Nutritious well-balanced meals are prepared with a connoisseur touch, and an opportunity to thrive in a safe and secure environment, surrounded by daily activities which help sustain their dignity and independence.
